Zusammenfassung
Krankheit und das Erleben eingreifender Therapie sind Welterfahrungen, de-
ren Auswirkungen auf das Seelenleben beträchtlich sein können. Dass
schwere körperliche Erkrankungen und darauf abzielende intensivmedizini-
sche Krankenhausbehandlungen vielfach mit erheblichem Stress verbunden
sind, aber auch negative psychischen Folgen haben können, wie Depressio-
nen, Ängste, Süchte oder posttraumatische Belastungsstörungen, ist be-
kannt und etwa für Krebs- und Herzerkrankungen empirisch gut untersucht.
Von der Forschung weitgehend unbeachtet geblieben ist hingegen ein ent-
sprechender Zusammenhang für akute und chronisch verlaufende psychi-
sche Erkrankungen, insbes. schizophrene Psychosen, die der Autor als „facet-
tenreiche Multitraumata“ beschreibt. Auch bei dieser Art von Gesundheits-
störungen sind allein wegen der belastenden psychotischen Symptome (Pa-
ranoia), aber auch bedingt durch den Einsatz von Polizei und Feuerwehr bei
Zwangsunterbringungen sowie diverse repressive Vorgehensweisen wäh-
rend einer akutpsychiatrischen Behandlung, sekundäre Traumatisierungen
zu erwarten, die bei den betreffenden Patienten zu psychopathologischen
Symptomen im Sinne einer PTSD führen können. Akutabteilungen psychia-
trischer Krankenhäuser sind noch immer Orte, an denen Patienten nicht nur
dem (eigenen und fremden) Wahnsinn in konzentrierter Form ausgesetzt
sind, sondern auch Prozeduren und Umgangsformen zu ertragen haben, die
schwer belastend sind. Es handelt sich quasi um ein „Schutzraumtrauma“
(»sanctuary trauma«), das hospitalisierte Psychosekranke durchleben und
das iatrogene Schädigung hervorrufen kann. Der Beitrag erörtert komplexe
Zusammenhänge zwischen Psychose, Trauma und Traumafolgestörungen
unter diagnostischen und therapeutischen Gesichtspunkten.
Einführung
Folgender wesentlicher Zusammenhang soll – auf den Punkt ge-
bracht – in dieser Arbeit verdeutlicht werden: Wenn Menschen an

Mittlerweile belegen mehr als 70 psychoonkologische Studien die psychi-
sche Durchschlagskraft der Feststellung bösartiger Tumoren sowie nachfol-
gender eingreifender Behandlungen für Betroffene (vgl. Gunkel, 2004). Krebs

HIV/AIDS
Eine andere inzwischen hinsichtlich ihrer traumatogenen Potenz mehrfach
untersuchte, chronisch-progrediente und bislang tödlich endende Krankheit
ist HIV/AIDS (ca. 16 Studien, vgl. Gunkel, 2004). Ein konkretes traumatisches
Ereignis ist hier – abgesehen von der Diagnosestellung – nicht spezifizierbar,
dennoch zeigen Befunde einer kalifornischen Studie, dass fast die Hälfte
(42%) betroffener Frauen das Vollbild einer PTSD und weitere 22% aktuell
eine partielle Belastungsstörung aufweisen (Martinez et al., 2002). Dabei ist
die Symptomatik besonders verbreitet unter Personen mit traumatischer
Vorschädigung und/oder geringer sozialer Unterstützung. Auch bei männli-
chen Infizierten sind rd. 50% von PTSD betroffen; wobei die Angst vor dem
Tod eine zentrale vermittelnde Rolle spielt (Safren et al., 2003). Das Auftreten
einer PTSD-Symptomatik ist mit der Tendenz verknüpft, eine lebensverlän-
gernde hochaktive antiretrovirale Therapie zu vermeiden (Delahanty et al.,
2004) und auch anderweitig riskantes Verhalten zu zeigen (Hutton et al.,
2001; Brief et al., 2004).

Studien (Mollica et al., 1998; Snow et al, 1988; Shore et al., 1986; Chung et al.
2004).
Allerdings sind diverse Studien entgegenzuhalten, in denen nicht oder nicht
nur objektive Dimensionen der traumatischen Exposition (Intensität, Fre-
quenz, Dauer) einen signifikanten Vorhersagewert hinsichtlich Chronizität
und Schwere nachfolgender Störungen hatten, sondern auch oder vor allem
subjektive Größen, insbesondere die persönlich erlebte Gefährdung. Auch
ohne unmittelbare Konfrontation mit einer Gefahrenquelle oder ohne tat-
sächlich vorliegende vitale Gesundheitsgefährdung kann die individuelle Be-
wertung der Belastung hoch sein und die Intensität von Todesangst errei-
chen (Taieb et al., 2003; Schnyder et al., 2000). Eine Studie an (n=368) Be-
wohnern der beiden Ortschaften Novi Sad und Zrenjanin, in deren Nähe 78
Tage lang Nato-Bombardements stattfanden, ergab zwar bei 63,5% ausge-
prägte Symptome einer PTSD (Kapor-Stanulovic & Zotovic, 2001), jedoch
auch, dass der verwendete objektive Stressparameter (Entfernung zwischen
Aufenthaltsort und Bombenabwurfpunkt) im Gegensatz zur subjektiven Be-
wertung der eigenen Bedrohung kaum mit dem Ausmaß späterer Störungen
korrelierte. Fehlzeiten nach einem schweren Autounfall sind einer Schweizer
Studie zufolge zwar auch abhängig von der Art und Schwere stattgefunde-
ner Verletzungen, in erster Linie aber von subjektiven Bewertungen, welche
das Opfers (entwickelt) hat über die Schwere seines Unfalls sowie eigener
Möglichkeiten, damit zurechtzukommen bzw. Einschätzungen späterer be-
ruflicher Folgen (Schnyder et al., 2003).
Dass eine aversive Erlebnissequenz traumatische Qualität gewinnt, hängt
demnach maßgeblich von der Subjektivität des Erlebens ab, oder wie Cheryl
Tatano Beck (2004) es mit Blick auf eine von Außenstehenden als unspekta-
kulär erlebte Kindsgeburt formulierte, konstituiert sich ein (Gebär-)Trauma
im persönlichen Erleben der Mutter („in the eye of the beholder“).

Intensivstation
Empirisch erwiesen ist, dass der Aufenthalt auf einer Intensivstation nicht
nur Ängste hervorruft, sondern auch posttraumatische Belastungsstörun-
gen induziert (27,5% bei Schelling et al., 1998; 38,9% bei Schelling et al., 1999;
18,8% bei Stoll et al., 2000; 3 Monate nach Entlassung 14% PTSD + 8% parti-
elle PTSD bei Cuthbertson et al., 2004; 15% PTSD + 13% partielle PTSD bei
Scragg et al., 2001). Die betroffenen Patienten berichten von bizarren, hor-
rorhaften Alpträumen (64%), intensiven Ängsten (42%), Atemstress (38%)
und starken Schmerzen (40%) (vgl. Schelling et al., 1998), manche auch von
Panikgefühlen (Stoll et al., 1999) oder paranoid-wahnhaftem Erleben (z.B. der
Eindruck, die Krankenschwester wolle den Pat. umbringen), was verursacht
sein kann durch Medikamente, Schlafstörungen bzw. hypnagoge Halluzina-
tionen (dazu Jones et al., 2001). Seltenere Ereignisse, die bei zu geringer Se-
dierung schwer belastend sein können, sind das Miterleben von Todesfällen
und Wiederbelebungsmaßnahmen bei Mitpatienten. Bei Intensivpatientin-
nen nach einem septischen Schock hatten – wenn man einer postalischen
Nachbefragung trauen kann – immerhin 66% eine PTSD ausgebildet
(Schelling et al., 1999). In einer retrospektiven Studie der gleichen Forscher-
gruppe (Kapfhammer et al., 2004) ergab sich an einer selegierten Stichprobe
für den Zeitpunkt der Entlassung aus einer Intensivbehandlung nach ARDS
eine PTSD-Vollbildrate von 43,5% (plus 8,7% subklinische Fälle), während
beim 8-Jahres-Follow-up noch Störungsquoten von 23,9% (plus 17,4%) zu
diagnostizieren waren. Nelson et al. (2000) fanden zwar einen statistischen
Zusammenhang zwischen der Anzahl von Tagen, an denen ein Patient nach
einer Lungen-OP auf der Intensivstation sediert worden war (=Indikator für
die Traumaintensität), und der Ausprägung von posttraumatischen Störun-
gen 6-41 Monate später. Allerdings halten es die Autoren für möglich, dass
ohnehin ängstlichere und von daher für die Ausbildung einer PTSD prädesti-
nierte Patienten häufiger sediert wurden. Dass weder die Dauer einer Un-
terbringung auf der Intensivstation, noch die tatsächliche Krankheitsschwe-
re als Prädiktoren für PTSD bedeutsam sind, wohl aber die Dauer künstlicher
Beatmung, zeigte eine Studie von Cuthbertson et al. (2003). Die Daten von
Kapfhammer et al. (2004) bestätigen die geringe prognostische Bedeutung
der „objektiven“ Krankheitsschwere, sprechen aber mit Blick auf PTSD
durchaus für einen ungünstigen Einfluss der Dauer intensivmedizinischer
Betreuung.

„(...) Wieder geht die Tür auf, noch mehr Männer und Frauen kommen
herein, alle stürzen sich auf mich, drücken meinen Körper fest aufs Bett,
halten mich eisern umklammert an den Händen und Beinen fest, einer
kniet auf meinem Unterarm. Kommandos werden ausgetauscht. Ich habe
nackte Angst. Sie werden mich töten, das weiß ich. Die Menschen kennen
kein Erbarmen, sie reagieren überhaupt nicht auf mich, sie werden mich fol-
tern und vergewaltigen.
Einige haben weiße Kittel an. Sind es Organdiebe oder aber bösartige
Wissenschaftler, die mir einen Chip ins Gehirn einpflanzen, um mich fortan
als Marionette durchs Leben gehen zu lassen? Ich kann kaum atmen. (...)
Bitte, Gott, mach dass es schnell vorbeigeht. Lass sie mich umbringen, ohne
mich vorher zu vergewaltigen. Ich denke an meine Kinder. Ich werde sie nie
wiedersehen. Ich weine und bete. (...)
Eine Frau schlägt die Bettdecke zurück und drückt die Oberschenkel aus-
einander. »Ich werde sie jetzt katheterisieren«, sagt die andere und streift
sich umständlich sterile Handschuhe über. Sie will mich umbringen, ich
weiß es. (...) Ich schreie und wimmere so lange, bis mir eine Spritze in den
Oberschenkel gestoßen wird, die mich in einen ungnädigen Psychophar-
maka-Halbschlaf fallen lässt.“

kann auch Verleugnung, die eine Folge von Schamgefühlen ist, die Registrie-
rung eingreifender Maßnahmen unmittelbar nach deren Anwendung mit-
bestimmen, wie Smolka et al. (1997) zeigen konnten. In deren Studie vernein-
ten direkt nach einer Zwangsmaßnahme 25% und gegen Ende der stationä-
ren Behandlung sogar 42% der Befragten, während des Aufenthaltes einer
Maßnahme gegen ihren Willen unterworfen gewesen zu sein!
Potentiell traumatogene Situationen, denen Patienten im Psychiatri-
schen Krankenhaus begegnen, sind vielgestaltig und höchst indivi-
duell, zumal stets der unkalkulierbare Faktor Mensch beteiligt ist.
Stark ängstigend und angesichts einer ohnehin fragilen psychischen
Integrität besonders beeinträchtigend ist gerade für erstmals mit ei-
genen psychotischen Symptomen konfrontierte Patienten die raum-
zeitliche Nähe zu anderen Kranken. Diese stecken mitunter noch
heftiger als der Patient selbst in den Wirren ihres Wahns, imponie-
ren durch Verkennungen, bizarre oder skurrile Verhaltensweisen,
akute Erregungszustände oder auch durch ebenso ausgeprägte wie
erschreckende extrapyramidalmotorische Nebenwirkungen der Neu-
roleptika (Frueh et al., 2000). Das hexenkesselartige Milieu einer
akutpsychiatrischen Station kann für unerfahrene, erstmals hospita-
lisierte und hoch vulnerable Psychotiker durchaus schockierend sein,
insbesondere wenn der Kontakt zum Pflegepersonal, zu Ärzten und
zu anderen Therapeuten durch geringe Empathie bzw. einen kühl-
distanzierten Schematismus oder anderweitig unprofessionelles
Verhalten im Umgang mit dem Patienten charakterisiert ist.
In hohem Maße eingreifend, oft mit dramatischen Umständen ein-
hergehend und damit stets belastend sind jedoch Fixierungen, die
von Patienten deutlich unangenehmer erlebt werden als etwa die
zwangsweise Verabreichung von Psychopharmaka (33% vs. 17%,
Naber et al., 1996). Das gilt in erster Linie für den betreffenden
fremd- oder autoaggressiven Patienten, der fixiert wird, d.h. gegen
seinen Willen mit Gurten an sein Bett geschnallt wird, es gilt teilwei-
se aber auch für Pflegemitarbeiter, die eine Fixierung durchführen.
Mechanische Bewegungseinschränkungen zum Zwecke der Ruhigstellung
aggressiver, agitierter und extrem unruhiger Patienten, sind dokumenta-
tions- und bei gehäuftem Einsatz auch (per Einwilligung bei einsichtsfähi-
gen Personen, ansonsten per Gerichtsbeschluss) genehmigungspflichtig, da
sie sonst als Freiheitsberaubung einzustufen und damit strafbar sind. Eine
Fixierungsmaßnahme ist z.B. bei randalierenden und andere Personen an-
greifenden Patienten im Sinne der Notwehr gerechtfertigt, aber auch hier
ärztlich anzuordnen. Der Einsatz von Fixiergurten als letztes Mittel zur Ver-
hütung eines zu erwartenden Schadens, zur Abwendung erheblicher, unmit-

telbar drohender Gefahren oder zum Schutz eines Kranken, bei dem Eigen-
oder Fremdgefährdung vorliegt, basiert auf dem sog. rechtfertigenden Not-
stand (Großkopf, 2004). Zu unterscheiden ist das Angebundenwerden vom
Angebundensein. Selten wird nämlich bedacht, dass über den Schrecken
und die Dramatik des Fixierungsaktes hinaus die fortan gefesselte Person in
jeder Hinsicht und mit zeitlich unklarer Perspektive mit sich selbst allein ge-
lassen bzw. vom beaufsichtigenden Pflegepersonal abhängig ist. Das ruft
häufig existenzielle Befürchtungen hervor, z.B. wie ein eingesperrter Hund
seine Notdurft nicht selbständig verrichten zu können bzw. in der Fixierung
rettungs- und schutzlos einem Zwischenfall – z.B. einem Brand, Angriff oder
sexuellen Übergriff durch Mitpatienten – zum Opfer zu fallen (Johnson,
1998, pp. 197-201).
Abgesehen von medizinischen, ethisch-moralischen oder rechtlichen
Gesichtspunkten, stellen Fixiermaßnahmen, selbst wenn sie in eine
bestehende therapeutische Beziehung eingebettet sind, psycholo-
gisch einen traumatischen Vorgang dar, bei dem Betroffene Ohn-
machtsgefühle und Todesängste entwickeln können. Zudem können
Fixierungen reale traumatische Vorerfahrungen reaktualisieren
(Smith, 1995; Frueh et al., 2000; Cusack et al., 2003; Jennings, 1994)
oder auch deren Reenactment darstellen (Gallop et al., 1999) und da-
bei psychotische Exazerbationen auslösen. Einer Fixierung ausge-
setzte Patienten sind trotz gezeigter Aggressivität vielfach emotio-
nal sehr verletzlich (Cohen, 1994) und erleben sich selbst dement-
sprechend in den Ledergurten als vollkommen hilflos und jedem
körperlichen und medizinischen Zugriff ausgeliefert. Oft empfinden
sie das Festgeschnalltwerden als unnötige Zusatzbelastung, gele-
gentlich (teilweise auch zurecht) als psychische und körperliche Be-
strafung für vorherige eigene Gewaltausbrüche (Richardson, 1987;
Soliday, 1985; Tooke & Brown, 1992; Soloff et al., 1985) und nur sehr
selten als Sicherungsmittel bzw. Begrenzungshilfe. Auch die von
Pieters (2003a,b) mit gerichtlich untergebrachten Patienten durchge-
führte Befragung zeigte, dass Fixierungen als hochgradig belastend
erlebt werden, weil die Betreffenden hierin eine Demonstration von
Macht bzw. ein Disziplinierungsmittel sehen.
Dutzende weiterer Untersuchungen (z.B. Johnson, 1998; Meehan et
al., 2002; Norris & Kennedy, 1992; Sagduyu et al., 1995; Roe et al.,
2002; Petti et al., 2001) belegen, dass repressive psychiatrische Maß-
nahmen von Patienten durchweg negativ bewertet werden, weil sie
Hilflosigkeit, Ärger, Traurigkeit, Ohnmacht und Frustration implizie-
ren. Den Mitarbeitern des Pflegedienstes, die im übrigen gelegent-

Auch die Isolierung eines Patienten, die hierzulande nur noch selten
vorkommt und der Minimalisierung des Aktionsraumes, aber auch
dem begrenzten Ausleben von Handlungsimpulsen dient (Hrachovech,
2004), kann erhebliche seelische Belastungen hervorrufen (Hammill
et al., 1989; Soloff et al., 1985). Früher erfolgte die Separierung oft
durch vorübergehendes Einsperren in sog. Gummizellen (auch als
Tobe- oder time-out-Räume bezeichnet), wo sich einerseits aggressiv
gespannte Psychotiker ohne Selbst- oder Fremdgefährung körperlich
ausagieren konnten, während heute bei der Isolierung eher die Reiz-
abschirmung in einem stimulusarmen Klima im Vordergrund steht.
Dennoch erleben viele Patienten diese Form der Absonderung von
der Stationsgemeinschaft als Einzelhaft.
Grundsätzlich stellt die unerwartete Zuführung in eine psychiatri-
sche Klinik und die dortige Aufnahme für einen hiervon überrasch-
ten bzw. überrumpelten, akut kranken Menschen einen Schock dar.
Abgesehen davon ist auch die zwangsweise, d.h. unter Einsatz von
Polizei, Feuerwehr und unter Beteiligung eines Gerichts (sog. Unter-
bringungsbeschluss) durchgeführte Hospitalisierung als traumati-
sierender Faktor zu betrachten (Beveridge, 1998), wenn man die Aus-
sagen von Patienten subsummiert, die in Gesprächen – oft spontan –
hiervon als schrecklichem Erlebnis berichten. Im Verlaufe einer Kli-
nikunterbringung, die ohne eigene Zustimmung vollzogen wird,
spielt oft auch reaktive Aggressivität des betreffenden Patienten
eine Rolle, was wiederum die Gefahr einer gewalttätigen Eskalation
im Kontakt mit Ordnungshütern, Sanitätern oder Klinikfachpersonal
und damit auch die Wahrscheinlichkeit des Einsatzes von Zwang er-
höht. In der Studie von Jackson et al. (2004) gaben immerhin 63% der
(n=35) erstmals hospitalisierten schizophrenen Patienten an, dass
bei ihrer Unterbringung die Polizei beteiligt gewesen wäre; 29% wa-
ren von einer gerichtlichen Anordnung betroffen und 40% kamen
(im Mittel für 32 Tage) auf eine geschlossene Station. Die dort absol-
vierte Behandlung beschrieben 82% als ziemlich oder sehr belastend
und 77% aller Befragten bewerteten die erste Erkrankungsepisode
als extrem stressig!
Andererseits zeigen zwei empirische Studien, von denen eine vom
Autor selbst durchgeführt wurde, eindeutig, dass die Häufigkeit bzw.
Intensität aktueller posttraumatischer Belastungsstörungen bei
chronisch schizophrenen Patienten mit zurückliegenden, selbst er-

6) Ein ehemaliger Patient beschrieb seine eigene, vor Jahren erlebte Fixie-
rung als derart einschneidend, dass er nicht nur die Gesichter der betei-
ligten Pfleger immer noch präzise vor Augen habe, sondern auch der
Geruch von Leder bei ihm intensive, emotionsgeladene und kaum be-
herrschbare Rückerinnerungen an Fixiergurte hervorrufe. Außerdem hät-
te ihn das Entsetzen über das Eingesperrtsein und die schmerzhafte Fes-

kommen ist, sollte dies zum Anlass genommen werden, alles mit
dem Patienten ausführlich zu besprechen, falls er dazu bereit ist.
In Ermangelung ausgearbeitete Konzepte und Therapieverfahren für
eine wirksame und gefahrlose Bearbeitung von Krankheits- und Be-
handlungstraumata schizophrener Patienten, kann man lediglich re-
kurrieren auf wenige Therapieansätze für psychiatrische Patienten,
die körperlich bzw. sexuell missbraucht wurden oder Zeuge von Ge-
walt geworden sind (Harris, 1998; Rosenberg et al., 2001). Grundsätz-
lich gilt jedoch, dass eine kreative und auf die Person eingehende
Grundhaltung nützlich ist, um brisante Inhalte mit gebotener Vor-
sicht ansprechen oder aufzugreifen zu können. Mit Konfrontation ist
dabei sparsam und dosiert umzugehen. Damit aversive Erfahrungen
schließlich in das Lebensskript integriert werden können, sollte die
Arbeit mit sekundär traumatisierten Schizophrenen auf dem Grund-
satz anzustrebender seelischer Homöostase bzw. Salutogenese ba-
sieren, aber auch auf dem immer präsenten Wissen um die spezielle
Gefährdung dieses Klientels (Vulnerabilität, Dekompensationsanfäl-
ligkeit). So gesehen lassen sich Prinzipien wie Ermutigung oder kog-
nitives Reframing als stabilisierende Komponenten einbeziehen.
Umfassende Behandlungsmaßnahmen im Sinne einer regelrechten
Traumatherapie sekundärer Störungen werden nicht zuletzt wegen
der komplizierten psychotischen Primärerkrankung in der Regel
nicht zum Tragen kommen. Eventuell trägt ja auch die Beherzigung
des eklektischen Grundsatzes „Was helfen könnte, solle man versu-
chen“ dazu bei, neue Wege der Sekundärprävention zu finden.
