Discharge Planning

General condition of client upon discharged : During nurse patient interaction upon discharge, the patient was wearing a comfortable clothes and a pair of flat slip-ons. Have a good bonding with her baby. She was oriented enough to follow instructions and answers questions asked by the student nurse.
