How To Use Google Docs Form in Conducting Multiple Choice Type Survey

Open Google

Click More

Click Documents

This will appear

Click SIGN UP if you have no Google account

And register for a Google account

Sign In if you have a Google account

Google Docs account will appear

Click CREATE to create a document

Click Form

The Google Docs blank Form will appear

Type in the Survey title

Type in the Survey title

Type in what you wanted to say about the survey

Type in the Survey question

Leave the Help Text box blank

Type in the Question Type (we will use Multiple choice)

Type in the choices

Click “Make this a required…” box

Click Done

This will appear

Click Theme to give a distinct look on your survey

Variety of themes will appear

Select a Theme (we will use “Swimming with the Fishes”)

This will appear

Click Apply

Click Apply

This will appear, Theme: “Swimming with the Fishes”

Click Email this form to send the survey

How to Send the Survey

Click Email this form to send the survey

This will appear

Add the address of people that you will survey

Add the address of people that you will survey

Click “Include form…” box

Click Send

Email will be send to your contact

Your contact will answer it

He will click submit

Google prompt will appear after clicking Submit tab

How to Check Survey Result

Check survey result by clicking See responses

Check survey result by clicking See responses

Click Summary

Survey result will appear in pie chart and coordinates…

Click Spreadsheet

Survey result will appear in Spreadsheet

How to Share Survey Result

Click Share on the upper leftmost of Spreadsheet

Click Share on the upper leftmost of Spreadsheet

This will appear

Share the data by adding emails in ‘Add people’ box

Click Share & save

Shared person email address will appear on “Who has access”

How To Add Questions on the Survey Form

Add a question by clicking Add item

Click Multiple choice (sample only)

Another Question box will appear

Fill in Question Title

Fill in Question Type

Fill in the Choices

Click Done to add the question in your survey
