San Rafael, CA 94901 May 07, 2012 Barbara Jennings Manager Handy Dandy Insurance Company 440 Montgomery St. San Rafael, CA 94901 Re: Denial of Insurance Claim Claim Number: 123456 Policy Owner: Dorian Mayhew Rothschild Dear Barbara Jennings: On March 22, 2012, a claim was filed with you regarding damages sustained from an auto accident I suffered on March 3rd, 2012. I want the insurance company to cover damages to my car which resulted from that accident. On April 27, 2012, I received notice that the claim was denied. This notice was received in writing. A copy of the notice is attached for your reference. I am writing this letter because the denial failed to include an explanation. Please reconsider your denial of the claim and provide an appropriate explanation. You may contact me at the above address if you have any questions or need additional information.
