1.0 SCOPE : The primary objective of this instruction is to prevent any work on energized equipments. To achieve such objective each and every electrical lock out permit has to be connected with a work permit and both documents have to be managed in a co-coordinated manner. The scope covers all areas in the plant (Ops area, Non-ops area, Jetty area and any other area where electrical lockout is required which is connected to Plant). 2.0 SAFETY: 2.1 All Instructions as listed in work permit should be followed. 2.1 No activity shall be started on respective equipment before the electrical lock out permit section B is filled and signed by the electrical shift in charge. 2.3 No activity shall be resumed after the electrical lock out permit section C is filled and signed by the OSI. 3.0 PROCEDURE DETAILS Responsibility Sr NO 3.1 Specific Tasks If a work permit requires an electrical lock out to perform the task safely, initiation for electrical lock out permit from Main control room is required. Operation should get two copies (white and yellow) of electrical lock out permit Performa and completely fill up section A on both yellow and white copies of permit Performa to request for de-energizing the equipment. Concerned PLL dept. representative requesting the job should sign the permit with date & time under section A – signature of maintenance representative. Equipment name should be clearly mentioned in permit. Overall responsibility for implementation of Electrical Lockout system is with Operation Shift Incharge (OSI). Any job requires electrical lockout, OSI will decide and take final decision in consultation with Executing dept. Executing dept. will request for electrical lockout in section A and sign the electrical lockout permit. OSI will verify all the details about the equipment to be locked out and its equipment number and sign the electrical lockout permit in Section A. Ensure that equipment is in off condition prior to sending the permit to substation. Equipment to be kept in manual mode in DCS before sending the electrical lockout permit to substation. Electrical lock out permit is sent to main substation from MCR (Both yellow & white copy of permit removed from book along with Padlock & Key). Padlock and Key is maintained and issued from Main Control Room.

One electrical lock out permit for multi-equipments is not allowed and each equipment needs separate electrical lock out permit Any work on main equipment, main equipment and its all auxiliary equipments to be locked out separately. Any work on auxiliary equipment, main equipment also to be locked out. If permit is raised by contractor, Concerned PLL department under whom the contract is awarded should sign the permit under signature of maintenance representative. Permit should not be issued to contractor directly. It should be issued to concerned PLL department under whom the contractor is working.

Operation shift in charge should sign under signature of OSI in section A of permit and send both Operation Shift copies (removed from book) to Main substation building Incharge Electrical shift in charge should fill up both white and yellow copies of section B of permit Performa and de-energize the equipment, properly fill electrical lock out tag and hang the first part of tag on equipment panel. He should sign under section B-signature of shift in charge (electrical) and register in electrical lock out register and assign electrical lock out number. Yellow copy of permit will be retained in file namely ‘Permits under job’ at main substation and white copy of permit along with filled second part of tag to be returned to main control room after tag number is written on the electrical lock out permit. After receiving the electrical lockout permit from MCR, Electrical shift Incharge should de-energize the panel and provide a padlock and Tag (first part) on the panel. Enter the padlock number, Tag no. and other details in Section B in both white and yellow copies of electrical lock out permit. Electrical shift Incharge should sign in Section B and enter the details in electrical lock out register namely ‘Permits under job’ kept at substation. Keep the yellow copy of the permit at main substation and send the white copy of permit along with the key & Tag (second part) to MCR. Substation Shift Incharge

White copy of electrical lock out permit to be retained in MCR and shall be entered in the electrical lock out register in the main control room. Enter the electrical lock out permit number in the main permit Performa such as hot work, cold work, vessel entry etc., After receiving the white copy of permit at MCR along with the key & Tag (second part), MCR handover the Tag (second part) to field operator and keep the physical key at MCR. Filled second part of tag to be hanged on local control panel by respective field operator. DO NOT OPERATE danger Tag also to be hanged on local panel at site by field operator before starting the job. Field operator should test the equipment at site using local start and confirms that equipment is Field Operator deenergised and press the emergency push button and field tag has been placed on the local start switch and then informs the same to MCR. Execution dept. also cross check the field equipment using local start and confirms that equipment Execution dept. is deenergised and field tag has been placed on the local start switch before starting the job. Wherever required, executing dept. may cross check the equipment lockout in front of field operator. After receiving the information from field operator that Tag has been placed at site, MCR enters Operation Shift the electrical lock out permit number in the main permit Performa such as hot work, cold work, Incharge vessel entry etc. and main permit number in white copy of electrical lock out permit and then issues the main permit to Execution dept. MCR keeps the key until the equipment is in electrical lock out. White copy of electrical lock out permit to be retained in MCR and shall be entered in the electrical lock out register in the main control room. Field operator should test the local start and confirm the same to MCR and field operator should allow the permit initiator to start the job only after local start is found negative. Before starting the job on the equipment, plant operator should sign the main permit and hand over Field operator to executing dept.

subsequent work on that equipment by execution department. Then permit execution department representative should sign under section C- maintenance representative with date and time on the white copy of electrical lockout permit. a) Send field operator to site to check whether the job has been completed properly and field Field operator operator informs to C/R about the status. Field operator releases the emergency push button and removes the field Tag (second part) and submit to MCR. If the equipment is having multiple lockouts, field operator should not release the emergency push Operation Shift button until all the lockouts are closed. He should sign under section D-signature of shift in charge (electrical) and make entry in electrical lock out register. White copy of permit to be kept in file namely ‘closed permits’ in main substation building and yellow copy of permit to be returned to main control room. White copy of permit to be kept in file namely ‘closed permits’ in main substation building and yellow copy of electrical lock out permit to be returned along with Padlock & Key to main control room. Electrical shift in charge should energize the equipment only after all the lock out permits (raised by various departments) for that equipment has been cleared. (Multi Lockout) Field operator checks whether local start/stop switch is in required operation mode. Field operator to remove the common Danger tag hanged on site local panel after he found job completed is normal.

MCR keeps the equipment in auto mode in DCS after completion of the job.

Operation Shift Incharge OSI should ensure complete electrical isolation of equipment in all respect before issuing main Operation Shift permit for safe working. Operation Shift Incharge While sending the electrical lockout permit for energising, ensure that main work permit is closed Operation Shift by executing dept. Incharge Electrical lockout permit to be retained in MCR for one month period from date of closing. Electrical lockout permit (yellow copy) to be retained in MCR for one month period from the date Operation Shift of closing. After the retention period, yellow copy to be shredded and disposed off. Incharge / Electrical lockout permit (white copy) to be retained in substation for one month period from the Substation date of closing. After the retention period, white copy to be shredded and disposed off. Shift Incharge One set of duplicate key is remaining with security department. If the padlock key is lost then Duty Security officer will issue the duplicate key after receiving the approval of MCR Shift Incharge. The padlock must be opened with duplicate key by following same existing lockout procedures.
