Crafters, Vendors, and Businesses are cordially invited to participate in the Providence Elementary Schools 2nd Annual Fall Festival

I hereby submit my application for the reservation of space, along with the $25 vendor fee, at Providence Elementary Schools Fall Festival to be held on October 27, 2012, subject to the terms and conditions detailed below:

PRODUCT DESCRIPTION: Please provide a detailed description (be specific) of the items/services (ex. Homemade baskets, makeup, jewelry, dance lessons) you intend to sell. Also, please note if electricity is needed. ______________________________________________________________________________

Please contact us before registering to ensure space is still available.

GENERAL INFORMATION:
Please make checks payable to: Providence Elementary School This is an outdoor event that will take place Saturday, October 27, 2012, from 4:00 pm-7:00pm. If it rains, the event will be canceled. Refreshments will be sold by the Providence Elementary School PTO and possible other vendors.

Return application (page one only) and entry fee (checks made to Providence Elementary School) no later than October 24, 2012. Please mail your application and check made payable to Providence Elementary School, c/o Kathy Mayfield, 287 Rainey Rd., Temple, GA 30179.

RULES:
Set-up will be from 3:00pm-4:00pm. Vendors may enter the circular bus driveway (where festival will take place) to quickly load/unload and may then park their vehicle in the adjacent school parking lot. Cancellations and Refunds: No refunds will be issued unless event is cancelled due to weather. Displays, tables, and chairs: Vendors must bring their own display materials, tables and chairs. Vehicles may not be parked on the lawn or sidewalks. Applications will be viewed and accepted on a first come basis. You are encouraged to submit your application as soon as possible. All participants will receive an email confirmation once we have received their completed application and fee.
