COMPANY Terms & Conditions Agreement

1. LAVAH services agree to service Company efficiently and effectively by supplying them with available, qualified, and responsible staff to help fulfill any of their various job needs and or requirements. 2. LAVAH services will be billing the signed agreed upon monthly Bill Rate. All Bill Rates encompass the cleaning service given to company, employees wage, vacation pay, all government burdens and remittances, WSIB coverage, overhead, advertising, applicant screening & testing, and mark-up. 3. LAVAH services assumes all related and expected responsibilities and expenses as the Employer, assuming any and all associated payroll costs applicable to our Staff. LAVAH services also reserves the sole right to establish the wages and fringe benefits of any of its employees, and assumes responsibility for the payment of such compensation, the withholding payment of all required payroll taxes, and the maintenance of Workers Compensation Insurance as required by Provincial and Federal Laws 4. Company agrees to comply with all federal, provincial and local laws, adhering to all ordinances and terms relating to the Ontario Labour Standard Codes & Canadian Occupational Health & Safety Act, and agrees to provide any safety equipment, clothing, and or required devices necessary and similar to that which Company own employees would use, as mandatory by law for any of the work to be performed properly and safely. 5. Company will not expose any of LAVAH services employees to unsafe working conditions, and will inform LAVAH services of any Potential workplace or work related Health Hazards, providing LAVAH services with all relevant MSDS information if applicable. Company also agrees to responsibly and proficiently train and make sure that all LAVAH services employees are aware of any policies and procedures relevant to their job, including all of the proper safety measures and procedures in case of emergency, and necessary job related training needed to perform their job functions properly and responsibly.

6. Company will report immediately to a LAVAH services Representative any workplace injuries no matter the seriousness of that injury, which may occur while LAVAH services employees are working on-site at any of Company locations. 7. Company agrees that it will NOT without the prior written consent from LAVAH services utilize or entrust any LAVAH services employees to operate machinery, equipment, and or vehicles not covered by Company own liability and property damage insurance. As well Company will not ask or allow any LAVAH services employee to operate any equipment or machinery that they are not qualified, trained and or currently licensed for. 8. Company acknowledges and understands that LAVAH services insurance does NOT cover claims of LAVAH services employees for damage to, loss of, or use of any Company owned, non-owned, and or leased vehicles (Including contents and cargo) machinery, equipment, and or material being used by or in the care, custody, or control of LAVAH services employees. 9. Company agrees that it will NOT entrust LAVAH services employees with an Unattended Premises, Cash, Cheques Negotiable, and or any other Valuables without prior written permission from LAVAH services. 10. NOT APPLICABLE -Company agrees that Overtime will be paid and billed at time and one half after 44 hours per week Company also agrees that if LAVAH services employees are required to work on a Public Holiday that they will be paid and billed at time and one half for that day as well. 11. Company is in agreement that they are NOT to hire any LAVAH services employees prior to the employee completing 528 hours of labour at your workplace 12. Company agrees to make all payments by month.

13. Company also agrees that they are NOT to disclose or communicate any part of this contract including pay and bill rate with any LAVAH services employees or any third party.

14. Company also agrees that they will NOT hire any LAVAH services even after 528 hours until all payments from previous issued invoices up to the 15 days agreed upon time allowance above is made in full. APPROVED COMPANY RATES
