CHECKLIST FOR GLOVING OPEN METHOD

1. Open the glove package aseptically. 2. Nurse prepares to glove herself. Tear open packet of powder and powder hands and discard. 3. With the left hand, grasp the right glove on the folded back cuff and lift directly up and away from the wrapper. Nurse steps back from the table. 4. Insert the right hand into the glove and draw it on, leaving the cuff turned well done over the hands . ( skin to skin technique) 5. Picking up the left glove, lift the glove from the wrapper by slipping gloved right finger under the protective cuff ( glove to glove technique) Avoid touching the gloved fingers to the bare wrist. 6. Pull the cuff of the glove up over the cuff of the sleeves. 7. Glove fingers of the right hand, touch only the outside of the left glove, near the wrist. Any exposed skin would contaminate the glove. 8. Place gloved hands on the wrist level. HOW TO REMOVE GOWNS, GLOVES, AND MASK? 9. Remove glove first. 10. Right gloves is partially removed by left gloved hand leaving fingers covered. Touch only the outside surface of the gloves. 11. Left hand glove is removed by placing still gloved fingers of the right hand on the cuff at the left glove without touching hand end of glove and pulling off. 12. Shake both gloves from right hand.

CHECKLIST FOR GOWNING

1. Circulating nurse opens the gown package aseptically. 2. Scrub nurse grasp the gown in one hand lifting it directly up from the package. 3. Scrub nurse steps backward away from the table. 4. Scrub nurseholds the neckband with both hands, gently shakes the folds from the gown. 5. Scrub nurse slips hand simultaneously in to the armhole, holding the hands upward on a level with the shoulder s. 6. Circulating nurse pulls the gown over scrub nurses hands, reaches inside the gown and grasps sleeve seam to do this. 7. Circulating nurse ties the gown at the back of the scrub nurse. 8. Scrub nurse leans slightly forward and swings the belt of the gown to the circulating nurse. Circulating nurse ties the belt at the back.

CHECKLIST FOR CHANGE OF DRESSING


1. Do the medical hand washing. 2. Explain procedure and prepare equipments. 3. Prepare the patient and position comfortably. 4. Wash hands and don sterile gloves. a. Old dressing is removed with sterile forceps. Use Zepheran chloride, normal saline or Hydrogen peroxide for dressings that stick to the skin. b. Discard and drop into the receptacle. c. The wound is then cleansed with a disinfectant or antiseptic of choice. Use a sponge only once, cleaning from up to bottom, then discard. Work away from the wound to a distance of about 2 inches, in only one direction. d. After cleaning irrigation follows ( if ordered for large wounds ), with the use of sterile syringe, irrigate the wound with a irrigating solution ( Normal saline solution ). Afterwards. Pat dry with sterile sponges. e. Place the sterile dressing over the wound using the sterile forceps. Drop in place. f. Secure dressing with adhesive tape or bandage.

CHECKLIST FOR OPENING A STERILE PACK OR TRAY AND HANDLING STERILE OBJECTS AND STERILE FORCEPS TECHNIQUE

1. Do the medical hand washing. 2. Open a sterile set or tray by unfolding the topmost part of the covering wrapper away from you. 3. Open the second layer of the wrapper to the sides of the set or tray. 4. Open the final layer of the wrapper toward you. The wrapper can now become the sterile field immediately surrounding the sterile set or tray. 5. In handling sterile objects, keep the prongs of the forceps together while removing it from the container and lift it without touching any part of the container. 6. Keep tip of sterile forceps always held downwards. Never hold it above in horizontal level. 7. Carry forceps high ( above waist ) and in sights, never swing it at your side. 8. Removing drops of solution from forceps by letting drops fall off over center of the jar. 9. Lift the lid of the container storing items by holding the lid down, to avoid contamination of the inner surface of the lid, or place the outer cover of the container over the unsterile field. 10. Do not return unused items to the container after removing them to avoid contaminating the other items in the sterile container. 11. Never touch the tip of a forceps to the nurses table when putting articles on it. 12. If the sterile item is individually package in an enveloped container, the top edge of the paper is opened and the forceps can be used to grasp the sterile item.
