I. ACKNOWLEDGMENT I would like to express my special thanks to my supervisors and co-workers that provided me the possibility to complete this portfolio. II. INTRODUCTION On the job training is one method by which students is given a chance to apply what they learned from the school. It also helps the student to acquire knowledge and skills by performing in actual work setting. It's a pleasure to be a trainee at Quezon City Health Department City Clinic because I have a chance to develop my skills. Im very satisfied here because I learned a lot and I experience new things.

OJT PORTFOLIO IV. OBSERVATIONS IN THE COMPANY What I have observed at the Clinic is they are still doing their job even they are tired and hungry. The strength and challenges of this company is that they are willing to serve the people even there's a lot of difficulties that they are encounter. The City Clinic will only serve the Quezon City residents so one of their challenges in the clinic is some of the patients that are not residents of the Quezon City will complaint in the QCHD head. I think the best practice of this company is I learned what are the things need to do if you are bitten by the cat, dog and rat and I also learned here how to interact with other people and know what are the different behavior of people. My over-all observation in this company is they are willing to serve and help people even there's a lot of difficulties that they are encounter and they do their job with smile on their face. V. PERIODIC REPORTS (daily, weekly, monthly) What we do daily are the following: Checking and listing the death certificates. Accommodating and assisting the patients. Recording and register the info of patients in the log book. Finding records. Taking the blood pressure of the patients. Cutting the cotton. Staple the ITR. Listing and giving the number to the patients. What we do weekly are the following: Filing records. What we do monthly are the following: Hypertension, Diabetes and TBR form template. During my whole training in Quezon City Health Department, I think the problem/s that I have encountered are giving the death certificate to the Doctor that not check or scan well, giving to other people the death certificate and forgot to record some of the info of the patients.

OJT PORTFOLIO VII. SELF-ASSESSMENT A. Skills and current technology learned/enforced: I learned what are the things need to do when you are bitten by the dog, cat and rat. I also learned how to use the electronic and manual blood pressure monitor and how to interact with different people. I used to multitask and focus my mind while doing a lot of things. B. Equipment, tools, testing apparatus handled (softwares used included) I used to handle the electronic and manual blood pressure monitor, and weighing scale.

C. Strong points versus weak points My strong points is my patience and my weak points are the irritable and ill-tempered patients. As much as possible I try to hold my angry but sometimes some of the patients pissed me off because they will blame you even if its their fault and there also some people will said bad words to you.

Best experiences on the job: My best experiences is interacting with people because I learned here what are the different behavior of people, why they are acting like that?. It opened my mind that not all people are the same, now I know why they are so irritable and ill-tempered so instead of become mad or say something bad to them Ill just understand them. E. Evidence of Background preparation: Faults committed (its causes and suggestion given by the supervisor Not scanning the death certificate clearly, giving the death certificate to other people and forgot to register some patients in the log book.
