1. Welcome and Introductions Dolores Colligan 2. Leadership Consortium Meeting-June 24, 2013, 2:30-4:30pm, GSC Rm. 250 Leadership Consortium Meeting-AGENDA ITEMS FOR ENDORSEMENT Dolores Colligan 2013 Continuum of Care Unified Funding Agency (UFA)

DCD will request approval from the Leadership Consortium to apply as the UFA for the 2013 CoC application: A handout was distributed summarizing UFA eligibility and guidelines. This would mean that HUD CoC funds would be allocated to DCD and then DCD would enter into grant agreements with community providers. In order to apply, a CoC must seek approval from its governing body Leadership Consortium. Out-of-County Residents Policy - Michael Hackman o o o o Initial thought is that it should be a guideline rather than policy Definition of resident includes anyone with an ID with a Chester County address, as well as a bill or lease with their name and Chester County address. Clarification around veterans seeking services: Veterans will be directed to the VA for services where this guideline does not apply, however if veterans are seeking shelter at a non-VA provider than this guideline will apply. There was discussion around the 5 day stay in shelter until transferred to appropriate services. There are concerns about allowing someone to enter into the shelter and then having to force them to leave. It was decided to change the wording eliminating shelter entrance, with some exceptions. o It will be important for Connect Points to understand this guideline in order to coordinate service for an individual in their County of origin ConnectPoints July 1, 2013 GOES LIVE! o The committee reviewed the Appendix A of the ConnectPoints contract Clarification is needed around how Connect Points will facilitate a web-based hub for information sharing, and also what type of advisory board will be established. (Gina Ruggieri to follow-up with Burroughs Mack, Family Service of Chester County) 3. Action Team Status Reports - Questions/ Discussion Systems Change Committee - Eric Forsythe o o o The next SCC meeting will take place on June 27, and the meetings will occur every 4 Thursday from 8:30-10:00AM Eric Forsyth has created a PowerPoint presentation to illustrate how ConnectPoints came to be as a system and process I&R agencies are invited, and this broader group of providers at this meeting will elect representatives for the SCC subcommittee

Housing Stabilization Action Team - Catherine Friedman o o Clarifi presented at the last meeting Catherine Friedman and Eric Forsyth decided that the Systems Change Committee and Housing Stabilization teams will not merge

Permanent Housing Action Team Dale Gravett o o o o o o The final survey has been sent out and already over 60 landlords have responded. A reminder to complete the survey has been sent to 600 Section 8 landlords Active planning for the landlord forum has started and the planning committee is hoping to hold the forum on a date in the first 2 weeks of September Virginia Supportive Housing is making a trip to Chester County in August to meet with developers with interest to partner with HACC on a project. HACC received an award letter for 35 additional VASH vouchers 3 developers have submitted applications to agree to set aside units for individuals and families experiencing homelessness. A decision on these applications will be made July 11, 2013 HUD issued a notice about helping to end homelessness in the county, and included guidelines for setting a preference for specific populations. HACC cannot have a separate list for families and individuals experiencing homelessness, but can set a preference, and open the waiting list to only receive applications from this set preference population. Also, application will not be accepted by self referrals, but by referrals from partnership agencies. Dale has to release a public notice regarding this preference and open waiting list, as well as complete a 45 day comment period before vouchers are released. o Gene added that all 3 rapid re-housing providers are in agreement to be the referral process for vouchers

The Fairview Village opening will not be on July 8. Dale will inform when a new date has been scheduled.

Results Action Team Dolores Colligan presented updates on behalf of David Weathington o o o David has been working on the ConnectPoints homepage, as well as establishing Call Point. The ConnectPoints administrators training is completed, and on 6/26/13 David will train the new Housing Support Specialists His Mission and Salvation Army have both transitioned their shelters into SRO units

D2D Updates Michael Hackman Migration of ReferWeb information to Community Points has been completed Systems Change Subcommittee: Once committee is selected and ConnectPoints is off the ground, the committee will begin to meet monthly ConnectPoints Provider meeting scheduled for June 21 at 9:00AM in GSC Rm. 170- invitations have been sent to provider Executive Directors and Program Managers. Community Networking Meeting scheduled for June 27 at 2:00pm at Kennett Friends Meeting House Rei Horst has distributed flyers throughout the Kennett Square area Legislative Briefing scheduled for July 11 at 8:30am in the Commissioners Board Room There will be a notice sent out by Friday to all legislators in Chester County and providers are also welcome to attend. Decade to Doorways 1 annual recognition event scheduled for November 21 at the Chester County Historical Society located in West Chester Hopeful for 200+ people. 1 or 2 providers will be recognized at this event as having embrac ed D2D concept. The Committee is responsible for charting detailed operational plans with specific actions, timetables, and resources and will be accountable to the Leadership Consortium. The Partnership Executive Committee is comprised of eight key members including representatives selected from the Leadership Consortium, Coordinating Agency, and the four Action Team Chairs. It will be chaired by the Coordinating Agency staff and act as the intermediary between the Coordinating Agency, Action Teams, and the Leadership Consortium. It will meet at least monthly. The Partnership Executive Committee is responsible for accomplishing the Plan, and furthering its goals by: Leading and managing the detailed implementation of the Plan, charting operational plans with specific actions, timetables, and accountability measures Providing assistance and guidance to the Coordinating Agency Preparing agenda, action steps, and other materials for Leadership Consortium meetings Recommending strategies, policies, and resolutions to the Leadership Consortium Producing annual reports and overseeing annual evaluation/updates of the Plan Facilitating collaboration and coordination between entities through memorandums of agreement Assigning tasks to and monitoring the work of the Action Teams Acting on behalf of the Leadership Consortium to address an emergency when the Leadership Consortium is not in session Developing the final recommendations to the Leadership Consortium for endorsement of the annual PA-505 Chester County Continuum of Care process on behalf of the Coordinating Agency
