Subject: Social Security Numbers

Attached please find a memorandum that Court Administrator Christopher
Leavey has disseminated.

We are in the process of eliminating the need for social security numbers on all
documents except for the claim petition, answer and the Second Injury Fund verified
petition. The social security number of a petitioner can therefore be left blank on the
current supply of forms other than claim petitions, answers and Second Injury Fund
verified petitions. This line will be eliminated on the applicable forms in future printings.
Petitioner social security numbers will also be eliminated on the judge’s copy of daily
lists.

This change is part of an ongoing security and identity theft prevention effort
affecting all State agencies.

Subject: Social Security Numbers

In our continuing effort to protect the security of our clients and the public, we are
removing petitioners' social security numbers from all documents filed after the initial
pleading in a case filed by each party. We are currently in the process of revising our
forms to effectuate this change.

Enclosed please find a Notice to the Bar indicating the change taking place and
advising parties that Social Security Numbers are no longer needed for all documents
filed after the initial pleading. Please post this notice in appropriate locations for
attorneys.

Because of this change, it is extremely important that the name of the party be
checked against the COURTS record when you are processing all documents. This will
be the only way to ensure the document is not misfiled because of an incorrect Claim
Petition Number.

Your assistance in implementing this new process will enhance the security of our
customers and reduce the chance of our documents being used to compromise a
petitioner's financial records.

NOTICE TO THE BAR

In an effort to ensure the security of Workers'
Compensation Petitioners, the Division is removing
Petitioners' Social Security Numbers fronl all docunlents
after the initial pleading filed by each party. Weare
currently revising all Division FOTITIS to reflect this change.
While this transition is pending, Social Security Nurrlbers
should not be utilized on any documents after the initial
pleadings.
