Hyperkeratosis, acanthosis, suprabasal acantholysis

Atypical, pleomorphic keratinocytes along the base, acantholysis, dyskeratotic cells

Marked basal keratinocytic atypia, acantholysis, clefting

Basal keratinocyte atypia, acantholysis, clefts, dyskeratotic cells

Acantholytic actinic keratosis

 

Site: Any chronically sun-exposed skin (head and neck, forearms, dorsal hands). Often multiple lesions. Acantholytic dermatoses such as Darier’s disease does not show basal keratinocyte dysplasia.
