APPLICATION FOR A TEMPORARY RESIDENT VISA – CHECKLIST
        Complete and place this checklist on top of your application. Consult our website for forms and instructions, including a list of countries whose nationals require a visa: www.philippines.gc.ca. Any document not in English or French must be accompanied by a certified translation. Do not include photocopies of your application package. You may keep them for your own reference. Failure to submit all required documentation may result in the refusal of your application or delayed processing. False statements or submission of fraudulent documents will result in immediate refusal. After submission of all required documents you may be required to attend an interview or medical exam. Any preparations you make or expenses you incur are done entirely at your own risk. Do not finalise travel plans unless you have received your visa from the Embassy of Canada to the Philippines.

You must submit the following: Completed “Application for a Temporary Resident Visa” (IMM5257) for each individual applying. If you and your spouse/common-law partner and dependent children are applying together, each family member must complete and sign a separate application form. Those applicants under the age of 18 must have a parent or a guardian sign on their behalf. Completed “Family Information” form (IMM5465) for all applicants 18 years of age or over. Correct processing fee. Fees must be paid by manager’s cheque, postal money order, certified cheque or bank draft payable to the “Embassy of Canada, Manila”. Cheques and PMOs should be valid for at least 6 months upon submission of your application. Cash will NOT be accepted and processing fee is non-refundable. Original passport(s) for yourself and each family member requesting a visa.  Each passport should have a minimum of two blank visa pages and minimum 6 months validity.  Attach old passports showing previous travel.  Children must have their own passport. Two passport photographs for yourself and each family member requesting a visa, with the name printed on the back, dated and taken in the past 6 months. Explain the purpose of your trip – Mention any other countries to be visited on your trip, other persons accompanying you, and any other information supporting your reasons for travelling. For convenience, you may use the Supplementary Information Form on our website. Original proof of funds – Updated original bank certificates with transaction history for at least the last 3 months (in the form of passbooks, bank statements, or time deposits). If you are going to visit family or friends, you need to provide your own proof of income and funds to demonstrate your financial situation in the Philippines. You may also provide the following:  A letter of invitation from your relative or friend that includes their full name, address, telephone and fax numbers, your name and relationship to the inviter, the intended duration of the visit, and a statement concerning who will pay for the trip expenses. These may include business correspondence and proof of previous business conducted (contracts. and financial documents such as certified business bank account statement for past 6 months.
