Gatekeeping Practices of Nurses in Operating Rooms Robin Riley, Elizabeth Manias Background of the Study/Rationale Lack of knowledge, lack of self confidence, lack of experience, and lack of trust of everyone to the team can influence everyones perception and insight about their team and the thought of them working together. Communication is a very important key for to the whole surgical team and communication is also the main reason why the team is having a hard time coordinating and working together with competency. Both of the study are focused on the communication of the health team members most particularly the nurse doctor relationship inside the operating room. Statement of the Problem Biblio 1: Will the 1-3 minute preoperative interprofessional team briefing with a checklist be effective to support communication in the OR? Biblio 2: Is the gatekeeping task of the nurses in the operating room a good way to maintain a sense of open communication and trust not only with the doctors/surgeons but also with the whole health care team members? Summary Biblio 1: Researchers observed 11 general surgeons and al members of the OR teams working in those surgeons including 116 OR nurses and 74 anesthesiologist, most OR nurses were assigned with different general surgeons and most anesthesiologist came from different institutions and services. The research was done in three tertiary hospitals in Toronto, Canada. The researchers focused on getting the data on the interpersonal skills of the health care team members in the operating room most particularly on the nurse-doctor relationship. Consent forms were signed by the health care team member for them to know that theyre being observed and all the data that are observed by the researchers will be used for their research. Biblio 2: Data were taken from three different operating room department, participants are composed of 11 operating room nurses who are tasked to be an informant by the researchers to report to them their cultural group and social environment place assignment. The researchers studied the view of the operating room nurses as gatekeepers and also studied the insights of the doctors with regards to the job of the nurses as gatekeepers. Findings Biblio 1: There are three different forms of silences in the operating room, absence of communication in the health team; not responding the questions and requests; and speaking quietly and not being understood by the health care team. the nurses makes sure that the instruments being used in the procedure in safe and sterile and the materials that are being used are according to the preferences of the doctors and if it will really save time and effort to the case. The operating room nurses are also the one who are tasked of bridging the gap between patients and doctors, and also asking the doctors before hand what they need, preferences, and style of performing the said procedures. Conclusion Communication is a very important key to success inside the operating room and it is also a way to exert yourself that youre not behind and youre not inferior to anyone you work for the patient and youre not under the doctors, nurses needs to assert themselves whenever they need to and nurses need to erase their belief that they cannot intervene with the procedure when they notice something wrong, nurses need to be assertive and be a patient advocate. Nurses should not be intimidated but they should be challenged and pursue more so they can assert themselves to the situation. We nurses should keep our data private and we must always remember that we're the one who's tasked to keep the documents and maintain the sterility of the items therefore we should do it with competence. Implications Nursing Practice This research helps nurses to improve more on their communication skills and be aware of the things that we should keep private and the things that we should keep sterile. Nursing Administration This research helps nursing administration in improving the relationship of the nurses towards their designated health care teams. Nursing Education This research helps clinical instructors in providing student nurses additional information on how to handle pressure in the operating room and how to assert ourselves to the situations and how we can manage to keep our documents safe and to how to keep our materials sterile. Nursing Research This research helps future researchers in studying on different ways on being assertive and the different nonverbal cues of the health care team members. Nursing Theory This research implies in the nursing theory of Patricia Benner; From novice to expert, because of its application to the nurses strive to become better so that they be competent enough to assert themselves and not feel inferior to the doctors.
