Victim was overcome by carbon monoxide inside restaurant by water heater in basement. Victim pouring floor stripper into bucket, bottle slipped from hands striking mop and forcing mop head into employee’s face. 2" stainless steel pipe ruptured on cooling tower return line - one employee killed by explosion, two seriously injured.
