PRIORITIZATION Problem Acute Pain related to tissue trauma status post Impaired Skin Integrity related to abdominal incision

Risk for Infection related to surgical incision Category Actual Justification This is the first prioritized Nursing Problem because according to Abdellahs Theory of 21 Nursing Problems, it falls under number 1 which is to maintain physical comfort. The patient recently underwent Total Abdominal Hysterectomy. Also, under number 3, which is to promote safety through prevention of injury or other trauma and through prevention of the spread of infection. The patient has a trans-abdominal wound. The pain is caused by the abdominal incision and once healed, pain will be gone, but there is still a risk for infection because of the surgical incision. This is the second prioritized Nursing Problem because according to Abdellahs Theory of 21 Nursing Problems, it falls under number 2, which is to promote rest and sleep. The patient is unable to sleep due to pain experienced from Total Hysterectomy. This is the third prioritized Nursing Problem because according to Abdellahs Theory of 21 Nursing Problems, it falls under number 2, which is to promote optimal activity. The patient is weak and unable to perform all activities of daily living.

This is the fourth prioritized Nursing Problem because according to Maslows Hierarchy of Needs it falls under Number 1 which is Physiologic Needs where sexual activity is a basic need. The patients last sexual activity was August 1, 2013. This is the fifth prioritized Nursing Problem because according to Maslows Hierarchy of Needs it falls under number 4 which is self-esteem where self esteem is affected by a disturbed body image. This is the sixth prioritized Nursing Problem because decrease peripheral tissue could cause patient to become drowsy which could lead to injury or other complications. This is the seventh prioritized Nursing Problem because if there is lack of peripheral tissue perfusion and if the patient s not yet fully recovered then it could lead to injuries.
