REMARKS
Survey
Following receipt of the notification of loss from the Consignee and your survey
instruction on 11th October 2013, we immediately contacted Mr. Sjaifu (the
Consignees Import Department staff) to arrange our survey.
On the scheduled date, 17th October 2013, we attended the above Consignees
warehouse where we met with and interviewed Mr. Sjaifu and Mr. Mulyadi Sirin (as
pharmacist). They then presented us to the affected cargo stored in their warehouse.
Mr Mulyadi informed us that they found the damage upon delivery to their
warehouse on 15th July 2013. They then sent the entire cargo to their customer on
22nd July 2013. However, the damaged cargo was rejected by their customer.
Some documents were handed over to us during our survey.
Nature and Extent of Damage
During survey we found that 3 tins @ 1kg of the cargo were dented.
Cause of Loss
Under investigation.
Remarks
We are collecting supporting documents from the Consignee and will report more
detail in the next Report.
