2014-2015 REGULAR BUDGET REQUEST FORM DIRECTIONS

Do not PRINT the form until the entire form has been COMPLETED! From the File Menu, select 'Save as' and save the form by the name of your student organization. Do not forget to continuously SAVE your form. Click on the 'Cover Sheet' tab on the bottom of this workbook and enter the necessary information in the cell directly above the 'requested information field.' Please note that you will not be able to type in many of the cells because they are protected.

Click on the 'Expense' tab on the bottom of this workbook and enter the necessary expense information. Please list requests in priority order and be as accurate as possible. Only short explanations should be given for each item. Additional explanation can be included on the "Explanation" tab.

After finishing the "Expense" tab similarly complete the "Income" tab. Then move on and fill the "Explanation" tab with any information you would like SFAC to know about your request. No additional information will be considered outside of these forms. An example request can be found on the Senate website. Save and print your form. Do not forget to print the Cover Sheet through the Detail sheet. Meet with your liaison and obtain all necessary signatures. E-mail a copy of the form to SFAC@drake.edu. The subject of the e-mail should be in the following format: '(Organization Name) Regular Budget Request 2014-2015' (e.g. Student Senate Regular Budget Request 2014-2015) You will receive a confirmation of receipt of your e-mailed form within 24 hours. Save this e-mail for future reference. Submit signed form and written documentation to the Student Life Center, Olmsted, by the deadline.
