Introduction

Guidance and Student Services Information System helps the admin records each

students offense by just clicking. Searching the students history would be an easy task too. The admin can also print the daily and monthly reports of the offenses to compare to others days or months. The system allows also the students to view their profile which they can also view their past offenses and the disciplinary action they must do. Purpose and Description This system will be prepared for the guidance counselor in organizing student records and student offenses as well as the summary of offenses committed daily and monthly. It will also be available for use of the head of student services, the students, and the student disciplinary board. Objectives This system aims to: Assist the administrator in recording students offenses Keep track of all the students records Be of convenient use to the administrator and students Improve the current student offense recording system

Important Information
Read this manual carefully For the convenient use of Guidance and Student Services Information System, be able to read the instructions carefully to acquaint the user with the system. Follow the steps and guides to understand the different functions and features of the system.

II. System Requirements


Hardware Specification The system will best run with an unlimited space of web hosting site and a computer with high-level performance; windows 7 and XP on the client-side workstation with at least 512MB of RAM and at least Pentium-class processor.

Software Specification Guidance and Student Services Information System is developed using PHP Programming Language, MySQL Server and/or Yii Framework. Web browsers: Latest versions of Firefox, Google Chrome, Internet Explorer, Opera, and Safari, to check that the page views runs across a variety of platforms. Adobe Photoshop for picture editing and graphics.

Log In This function will enable the user to log in as Student, Student Services Admin, Guidance Admin, or SDB Admin using their respective username and password.

User must choose from the Navigation Menu on the left and enter your username to the Username field and password to the Password field and press Enter or click GO to submit.

Student Side
Student Profile In this function, a student will be able to view his/her profile and edit information on his/her account.

To edit your profile, click on the Edit Profile link. After clicking the Edit Profile link, you will now be able to edit your profile.

Click on each text field to update a profile. Be able to fill in the required fields with truthfulness and clarity. After making sure all fields are correct, save the updated profile by clicking the Save Changes button.

Student Offenses In this menu, the student will be able to view their lists of offenses.

1. Click the Offenses button on the left menu to see your history of offenses. 2. Click the Previous Page link to view the previous list of offenses. 3. Click the Next Page to view the next set of offenses. 4. Click the Last Page link to view the last page of offenses. 5. Click the Log out button to finish session.

Student Reports In this function, students will be able to view his/her messages and report an incident to the Admins.

To report an incident, click the Submit an Incident to Admin link. To view a message, click View Message on the left side of the page. To reply to a message, click Reply on the left side of the page, beside the View message. To delete a message, click Delete at the left side of the page, beside the Reply. To navigate through the pages of the Reports tab, you can use the First Page, Previous Page, Next Page and Last Page navigators.

Student About In this function, students will be able to view information about Procedure for the Settlement of Complaints and Grievances and University Disciplinary measures.

Student Disciplinary Measures

In this function, student can view the Student Disciplinary Measures from the PUP student Handbook to acquaint them about the measures implemented in the school premises.

Student Settle For Grievances In this function, the rules and step by step process to settle grievances are listed.

Student Services Admin


Student Services Admin Notifications

In this function, the Student Services Admin can view his/her notifications on the Tagged Offenses and Messages. To view the Offense Record on the Tagged Offenses, click View Offense Record link on the left side of a record. To check messages, click View Messages link on the left to open a message. To reply, click Reply.

To delete a message entry, click Delete link at the left side.

To navigate through the page content, click Next Page link to proceed to the next page and click Previous Page to return to pages you had seen.

Student Services Admin Student Offense Record

Student Services Admin can view all the Records of the Student and perform functions such as View Offense Record and view the Test Records of each student.

In this function, the Student Services Admin can manage the offense record of a student. To add offense record to a student, user must select the Offense Name by clicking the drop down list and choosing from the list. Discipline Name is automatically selected upon entry of the Offense name. User can optionally put remarks on the offense record of the student. Select the date when it is comitted by clicking the Date Committed field. Click Save Changes to save the offense record. To view message from a student, click View Message button. To tag the offense record to the SDB Admin, click Forward to SDB Admin button. To tag the offense record to the Guidance Admin, click Forward to Guidance button. To print letter to parent to inform the of the offense records of their child, click Print Letter to Parent Link.

Student Services Admin Reports

The user will be able to view the Reports submitted by the students and filter it by the date of submission by entering the desired start date and the end date of the record and click Search button to filter records.

Student Services Admin Settings

In this function, user can manage the user accounts. To add user account, click Add Account link.

-Input desired Username and Password and click Add Account to save. To Edit a user account, click Edit link.

-Click on the field you wish to edit and change the content then click Edit Account button to save. To Delete a user account, click Delete link.

Guidance Admin

Guidance Admin Notifications In this function, the Guidance Admin can view his/her notifications on the Tagged Offenses and Messages. To view the Offense Record on the Tagged Offenses, click View Offense Record link on the left side of a record. To check messages, click View Messages link on the left to open a message.

To reply, click Reply.

Guidance Admin Student Offense Record In this function, Guidance Admin can view and manage all the Student Records and Offenses of the students.

To view the test record of a student, click Test Records link at the left.

To view the offense record of the student, click View Offense Record link at the left.

In this function, the Guidance Admin can manage the offense record of a student tagged to him/her by the other users.

To manage a record, click View/Manage button.

Guidance Admin will be able to view the offense records tagged to him/her by the Student Services Admin and the SDB Admin. On each records, user is provide with a Record Log to show the history/summary of records applied on a student record.

To tag the offense record to the SDB Admin, click Forward to SDB Admin button. To tag the offense record to the Student Services Admin, click Forward to Student Services button. To print letter to parent to inform the of the offense records of their child, click Print Letter to Parent Link.
