Experimental Advanced Renewable Program (EARP) Non-Residential Application and Instructions

(Note: This application is to be completed by customers applying for non-residential rate only.)

Please refer to these instructions when completing this Application. Not all fields are included in these instructions as most of them are self-explanatory. All applicable sections and fields must be completed in full for the application to be considered: 1. Customer / Account Information: This section should be completed using the same information as shown on the Account Holder’s (Customer/Applicant’s) Consumers Energy bill along with a Contact Name (if different than the Account Holder’s name). 2. Rate AR Tariff Information: To qualify for the non-residential AR tariff rate, the solar energy system must be owned or leased by a Consumers Energy customer and it must be constructed on or adjacent to a premises (owned or leased by applicant) where the applicant is currently receiving or will receive electric service from Consumers Energy under one of the following Consumers Energy rate schedules: RS1, RT1, REV1, GS, GSD, GP, or GPD. The system must be completely owned or leased by the applicant, with no third party having title or direct ownership or leasehold interest in equipment related to the solar energy system. If the solar energy system is or will be located on property or structures that are not owned by the applicant, the applicant must hold a lease, easement, or other instrument that provides the right to construct, own, and operate the solar energy facilities for a period of time that covers the contract term of the EARP Agreement for Service. 3. Generator System Information: Please enter information concerning applicant’s new (or existing) generator in ALL fields. Blank or partially completed fields in this section will result in your application being rejected. a. Nameplate Capacity and Annual Output fields: Enter the total generator nameplate capacity (in kW DC-STC) and expected annual production (in kWh) in these fields as accurately as possible. b. Percentage fields – Either (i) or (ii) must be met to be eligible: (i) Percentage Michigan Labor field: The generating system must be constructed with in-field labor by Michigan residents constituting no less than the following portion of total in-field labor hours: – 60% before 1/1/2013, or – 65% from 1/1/2013 through 12/31/2014, or – 70% after 12/31/2014 OR (ii) Percentage Michigan Equipment field: The cost of generating equipment and materials manufactured or assembled in Michigan must constitute no less than 50 percent of the cost of all generating equipment and materials comprising the solar generating system. c. Address fields: Please complete ALL address fields for the location of the proposed solar generating system including township and county. If the generator will not be located in a township, please enter ‘N/A’.

4. Existing Generator System Information: a. If the generator entered in prior section is not a pre-existing generator then please mark the ‘No’ box and continue to the next section. b. If the generator is a pre-existing generator and used to participate in Consumers Energy’s Net Metering or Experimental Advanced Renewable Program or, is a selfgeneration or Wholesale Energy supplier then mark the ‘Yes’ box in the first field and complete the remaining section. 5. Customer Signature: Please read and mark the boxes next to the statements to indicate acceptance. The electric service account holder whose name appears on the electric service utility bill MUST sign the application for it to be considered complete. Incomplete applications will not be considered a valid application until all requested information is provided. Information requested on the application is necessary to validate the application and to provide information that can be used for program analysis. NOTE: Applicants will be notified that the application has been received and the results of Consumers Energy’s review of the application2. If the application is determined to be ineligible due to the size, output, location or other significant characteristic of the system, a new application must be submitted prior to the application due date in order to be reconsidered for the given phase. Each participant provisionally awarded a portion of the capacity reserved for each phase will be notified by mail with an EARP provisional acceptance letter detailing the terms of program acceptance and the EARP rate that the applicant is qualified to receive. Consumers Energy will assume that EARP provisional acceptance letters are received on the third USPS delivery day after the acceptance letter is mailed. 6. Mail the application form to the EARP Coordinator at: Consumers Energy Company Experimental Advanced Renewable Program Coordinator 1945 W. Parnall Road, P13-304 Jackson, MI 49201 Applications must be delivered to Consumers Energy’s EARP Coordinator by 5:00 PM EDT on the Due Date listed at the top of the application to be considered for a particular phase. If sending by U.S. mail, allow adequate time for your application to be delivered by the Due Date. The application can also be sent via electronic mail to solar@cmsenergy.com.

I understand that submitting an application does not guarantee participation in Consumers Energy’s Experimental Advanced Renewable Program. I understand that upon receiving a provisionally awarded notification into the Experimental Advanced Renewable Program I will have ten business days to submit a Generator Interconnection Application and a partially executed Experimental Advanced Renewable Program Agreement for Service to Consumers Energy. I understand that payments under the Experimental Advanced Renewable Program will be based on the effective date of service, which will be determined in accordance with a fully executed Experimental Advanced Renewable Program Agreement for Service. I understand that Consumers Energy is not obligated to connect the proposed solar energy system by the Proposed In-Service Date specified in this application. I understand that the solar energy system installed for participation in the Experimental Advanced Renewable Program must deliver all energy not consumed by the solar energy system to Consumers Energy’s distribution system. I understand that the ownership of all emissions allowances and environmental attributes generated by the solar energy system (such as renewable energy credits) will be transferred to Consumers Energy under the terms of the Experimental Advanced Renewable Program Agreement for Service. I understand that all energy delivered under the Rate AR Tariff must be generated from the solar energy system and the solar energy system can not include any form of back up energy storage. I understand that in order to be eligible to participate, I must have a satisfactory payment history on my existing Consumers Energy account in the 12 months prior to the above Due Date.
