Follow the steps to enable your online Evaluation Form
1) Type the name of your manager whose performance you want to evaluate.
2) Check the performance categories and questions on the form. You can edit them if necessary. Also you may delete the questions and corresponding option
buttons that you will not use.

3) Visit the site below:
http://www.spreadsheetweb.com/getting_started.htm
You will only need the username and password to create your online evaluation form.
4) Visit the site below:
https://www4.spreadsheetweb.com/SpreadsheetWEB//
Login to page with your new account information.
5) Click "Add Web Application" to upload this file. Your online evaluation form will be created automatically.
You can simply use the form from that link or place it on your website.
6) Any time a form is completed and submitted, answers to the questions will be saved. The files will be saved with the name of the executive director and time
of form completion. You can simply reach the files using the "Data" tab. Saved files will be listed.
Also, changing the name of the director and uploading the file again, you can evaluate another director at the same time. Since the forms will be saved with
director names, you can create a collection of evaluation forms repeating the process.
