Orientation and Mobility Assessment

Background information
Kese is a 34 female who has Retinitis Pigmentosa and Optic Nerve Pallor.
Her vision has been deteriorating within a year. he has additional health
conditions s!ch as "!p!s and #idney fail!re. he receives dialysis three days a
wee#. Kese !sed to wor# as a $ertify N!rsing %ssistant !ntil her vision and health
started to decrease. he has a da!ghter who is &' years old and live with her. Her
sister and da!ghter assist her with shopping tas#s. he !ses comm!nity
transportation to get to her doctors appointments and dialysis. he has not
received rehabilitation services since she started to loss her vision d!e to
depression. (herefore) this is the first time that she contacted *ivision of +lind
ervices in order to receive s!pport in *aily "iving #ills and Orientation and
,obility.
Description of the testing environments and materials
-n order to obtain information abo!t her Orientation and ,obility s#ills) she
was assessed with the Orientation and ,obility initial eval!ation .!estionnaire from
-ndependence for the +lind. (he assessment was developed in her apartment and
apartment comple/.
(he -+ Orientation and ,obility .!estionnaire incl!des areas s!ch as how
the person travels at home and in the neighborhood. %lso) Kese had to specify if
she is able to ascend and descend stairs) and if she !ses any vis!al or mobility
aids to navigate from one place to another. %dditionally) it incl!des a list of methods
of travel s!ch as !naided vision) vision pl!s low vision aid) sighted g!ide) riding car)
cane and other) and the individ!al has to specify the fre.!ency that she !ses them)
the diffic!lty) ,oreover) Kese had to describe the characteristics of her vision in
different times of the day.
Findings
Kese demonstrated the ability to travel independently at home. he was
able to locate her #itchen) living room) and bedrooms. he was able to ascend and
descend stairs by holding the rail in order to go to the second floor. 0hen as#ed to
go o!tside) she located the door and went down a co!ple of steps. 0hen going
downstairs she tends to loo# at the gro!nd. (hen she located the sidewal# and she
wal#ed towards the mailbo/es witho!t a cane. he crossed the par#ing lot witho!t
ma#ing s!re if any car was coming. he traveled aro!nd her apartment comple/ by
only !sing her vision. he does not go o!tside d!ring the day beca!se she is
blinded by the s!ns glare) th!s she emphasi1ed that her vision is better on clo!dy
days.
Kese mentioned that she does not travel by herself in the neighborhood.
Her brother and sister pic# her !p to ta#e her grocery shopping or to ch!rch. Her
relatives hold her arm or sho!lder in order to assist her to travel. Kese !ses
comm!nity transportation to get to her doctors appointments and dialysis. %ltho!gh
she is able to see the traffic lights) sidewal#s and cars she does not feel safe
wal#ing witho!t her relatives. Her da!ghter helps her to perform different tas#s that
re.!ire traveling.
Recommendations
-t was recommended to !se a 24 white cane with rolling tip.
Kese re.!ires Orientation and ,obility services to learn how to provide
sighted g!ide instr!ction) cane s#ills) and street crossings.
0hen traveling with her da!ghter) instead of holding her hand) she will hold
her sho!lder in order to travel safely doing sighted g!ide.
0hen re.!esting help to travel in ch!rch) comm!nity) or her da!ghters
school) she will have to !se proper sighted g!ide.
0hen traveling to chec# the mail) she will have to travel aro!nd the
perimeter of the par#ing lot in order to get to the mailbo/es.
he will need to loo# at both sides if she needs to cross over the par#ing lot.
0hen going grocery shopping with her family) she will have to develop an
organi1ed list of items according to each isle in order to be familiar with the
store and to see# for more independence while performing this tas#.
