MISSING PERSON/RUNAWAY REPORT (TF-974)


Complete this report for missing persons or runaways who are Oakland
residents regardless of the jurisdiction in which they were last seen. An
Outside Agency report shall be completed for nonresident missing
persons who were last seen in Oakland. A Crime Report (536-933) shall
be completed whenever foul play is suspected.
MISSING PERSON
Any person who is missing under circumstances not conforming to his/her
ordinary habits or behavior or who may be in need of police assistance or
attention. Examples: the senile or aged person who is lost or helpless, the
mentally or physically handicapped, the blind, the epileptic, the person
known to be contemplating suicide. The police or sheriffs department
having jurisdiction of the place in which a person reported missing was last
seen is authorized to initiate the investigation.
A missing person call requires the exercise of the utmost tact by the
reporting officer. In many cases, it will be obvious that the adult reported
missing has actually left home of his/her own accord. The officer should
point this out to the reporting party and tactfully explain that the police
have no authority to act in such cases. However, if he/she indicates that
the missing person is likely to be a victim of foul play, or may be
contemplating suicide, take the report. Whenever in doubt, take the report.
The reporting officer shall:

Telephone the Youth Services Division immediately in all cases of


missing persons or runaways.

2.

Request a photograph of the missing person or runaway to be


attached to the report or to be mailed in later by the reporting party.
Be sure that the missing persons or runaways name and address are
written on the back of the photograph.

3.

Inform the reporting party that he/she must telephone the Youth
Services Division as soon as the missing person or runaway is located
or returns home.

4.

Notify the reporting party that upon the return of a reported missing
juvenile, the Missing Persons Detail requires an interview with the
subject and his/her parents.

appropriate, country.
Enter the date and time the missing person/runaway was last seen. Use
military format.
Enter the date and time the person was reported missing. Use military
format.
Enter the missing person/runaways probable destination, if known.

Describe the clothing worn by the missing person/runaway at the time of


disappearance, if known.
Describe any jewelry worn by the missing person/runaway at the time of
disappearance, if known. Describe any type of identification carried by the missing
person/runaway, e.g., student ID card, drivers license, passport, ID bracelet, etc.
Describe the physical condition of the missing person/runaway at the time of
disappearance at the time of disappearance.
Describe any salient physical characteristics of the missing person/runaway,
such as complexion, identifying marks or tattoos, dental work, etc.
Enter the missing person/runaways previous home address including building,
room or apartment number, and zip code. Enter the name of the city if outside
Oakland. If Oakland address, check the Oakland box.
Enter any significant personal habits of the missing person/runaway. Examples:
heavy drinker, gambler, etc.
If the missing person has a known history of disappearance, list the date(s) of
previous reports.
If the missing person/runaway has a known history of disappearance, list the
places at which he/she was previously located.
Enter the license plate number and state of the vehicle used by the missing
person/runaway. If there are no plates on the vehicle, leave blank.
Describe the vehicle used by the missing person/runaway. Fully describe the
vehicle including the license number and the state, the year, make, model, body
type, color and VIN. If completed information is not available, enter any partial
description. If the vehicle is a bicycle, fully describe the bicycle, including the type,
color, brand and model, number of speeds, and the license and serial numbers.
Enter the name of the missing person/runaways group/care home, if appropriate.
Enter the name and telephone number of the missing person/runaways social
worker, if appropriate
Check the appropriate box to indicate whether or not the missing
person/runaway is at risk due to medical condition, age, mental capacity, etc.
Enter the amount of money carried by the missing person/runaway at the time of
disappearance. Enter any possible reasons for disappearance. Examples: marital
trouble, despondent over debts, feared punishment, had quarrel.
Enter the name and telephone number of the missing person/runaways
probation/parole officer, if appropriate.

The narrative normally follows the order in which the events occurred. Include a
summary of any information which may have some bearing on the case, e.g.,
emotional history. Attach to the report any notes or letters left by the missing
person/runaway.

Enter your first initial, your last name and your serial number.
Enter the serial number of the second officer assigned to a two-officer unit. Do not
enter a cover officers serial number in this e.
