Guidance Notes for Completing UKAS Application Forms

Accreditation Standards
There are various standards that organisations can apply to be accredited to
by the United Kingdom Accreditation Service (UKAS).
We would recommend that potential UKAS customers obtain a copy of the
relevant standard that they wish to become accredited to for familiarisation
ahead of applying for accreditation.
All standards may be obtained from British Standards Online (BSOL):

If you are a certification body that wishes to be accredited to certify QMS*,
EMS*, ISMS*, FSMS*, BCM*, or H&SMS* then the appropriate standard is
ISO/IEC 17021:2006 (The appropriate AC form for applicant certification
bodies to complete is the AC1 [electronic version] / [hard copy version])
If you are a certification body that wishes to be accredited to certify Product
then the appropriate standard is EN45011:1998 (Same AC form as mentioned
above)
If you are a certification body that wishes to be accredited to certify Personnel
then the appropriate standard is BS EN ISO/IEC 17024:2003 (Same AC form
as mentioned above)
If you are a certification body that wishes to be accredited to certify EMAS*
then the appropriate standard is EMAS: Council Regulation (EC) No.
761/2001: March 2001 (Same AC form as mentioned above)
If you are an inspection body that wishes to be accredited then the
appropriate standard is ISO/IEC 17020:1998 (and the appropriate AC form to
complete is the AC2 [electronic version] / [hard copy version])
If you are a calibration laboratory that wishes to be accredited then the
appropriate standard is ISO/IEC 17025:2005 (and the appropriate AC form to
complete is the AC3 [electronic version] / [hard copy version])
If you are a testing laboratory that wishes to be accredited then the
appropriate standard is ISO/IEC 17025:2005 (and the appropriate AC form to
complete is the AC4 [electronic version] / [hard copy version])

If you are a proficiency testing organisation that wishes to be accredited then


the appropriate standard is ISO/IEC Guide 43-1:1997 (and the appropriate AC
form to complete is the AC5 [electronic version] / [hard copy version])
If you are an organisation thats seeking appointment by a UK competent
authority under relevant UK regulations or EC directives we would
recommend that you refer to UKAS Publication P16 (and the appropriate AC
form to complete is the AC6 [electronic version] / [hard copy version])
If you are an organisation that wishes to be an accredited Reference Material
Producer then the appropriate standard is ISO Guide 34:2000 (and the
appropriate AC form to complete is the AC7 [electronic version] / [hard copy
version])

Relevant AC form completed and the associated attachments required


Completed Main Application Form
Quality Manual (or equivalent management system documentation)
Articles of Association (if appropriate)
The supporting information required in Parts 1 and 2 of the Application
Form
Two signed copies of the UKAS Agreement (for new or first
accreditations with UKAS only. Not required for UK Regulations and EC
Directives)
Two signed copies of the UKAS Agreement Supplement (for new or
first accreditations with UKAS only. Not required for UK Regulations
and EC Directives)
A cheque (payable to United Kingdom Accreditation Service) for the
Application Fee of 1,200 + VAT (1410). (This fee is non-refundable.)

Certification Body Standards


There are various standards that organisations can apply to be certified to,
however, in order to obtain these you should contact a relevant certification
body from the UKAS-accredited certification body directory on the UKAS
website
(http://www.ukas.com/about_accreditation/accredited_bodies/certification_bod
y_schedules.asp). As an accreditation body UKAS is unable to certify to these
standards:
QMS ISO 9001:2000
EMS ISO 14001:2004
ISMS ISO 27001:2005
FSMS ISO 22001:2005
BCM BS 25999-2: 2007
H&SMS - OHSAS 18001:1999, OHSAS 18001:2007 & BS OHSAS
18001:2007
Extensions to Scope (existing customers only)
In order to apply for an extension to your existing scope of accreditation you
should complete and submit the appropriate AC form along with any other
documentation (Procedures, Quality Manual, etc) required to support your
application.
