I.

Please be informed that OOA 10:30 pm of August 04, 2009, Inflagrante


Delicto, 23 years old, single, residing at purok Tambis Barangay Umapad
Mandaue City. Was arrested by Velarde Demape, Zosimo Luage, Rofino
Monacillo, Elpidio Dechos, and Roquito Dionisio all by the board member of
barangay Umapad this City.

II.

Said suspect, was arrested Inflagrante Delicto sniffing and having in his
possession control and custody one plastic cellophane containing yellowish
substance believe to be a rugby.

III.

Said evidence now marked as JCM to be submitted at the regional crime


lab office.

IV.

Suspect temporarily detained at this office, ready for filing of charges in the
proper court. Progress report will follow. Requests acknowledge receipt by
the end.

II.

On or about 02:00 am, Saturday, Peter Galloza, 32, a security guard, killed
his brother-in-law Police Officer 1 Federito Armateo Jr., a member of PNP
Special Action Force who was assigned in Iligan City.

III.

In police investigation, it was showed that the victim was armed with two .
45 caliber pistols, shouting bad words against their family and forced his
way into the house of the suspect by destroying their door.

IV.

Recovered from the crime scene were two .45 pistols with live ammunition
and a gun recoil spring, while the unlicensed .357 revolver was not found
during the investigation.

V.

The suspect was examined to crime lab office, and the police personnel of
San Enrique are waiting for the result of the paraffin test.

VI.

The suspect is now under the custody of San Enrique Police Station for
the on-going investigation and for the awaiting result for the filing of the
case in proper court.

I.

Reference: PROGRESS REPORT re-homicide of PO1 Armateo

II.

On or about 02:00 am, Saturday, Peter Galloza, 32, a security guard, killed
his brother-in-law Police Officer 1 Federito Armateo Jr., a member of PNP
Special Action Force who was assigned in Iligan City.

III.

In police investigation, it was showed that the victim was armed with two .

IV.

The suspect is in full custody of San Enrique police station for the on-going
investigation and waiting for the filing of the case in the proper court.

It was showed that the suspect fired a gun to the victim due to self-defense
and because of its aggression and intoxication.

VII.

After the examination, the suspect was identified positive for firing of a gun
to the victim and charge with a case of homicide.

FACTS
Initial investigation conducted disclosed that on the above cited time, date, and
place, vehicle of Richard Gutierrez was travelling to Sta. Rosa Tagaytay Road in Silang
Cavite, and was driving on his way home. The car crashed into an electric post. As a result,
the vehicle, with passenger inside fell into the ravine and hit a coconut tree. Gutierrez and
his body guard, George Mastura were injured, but the actors assistant, Nomar Pardo, died
while being treated at the Adventist University of the Philippines Hospital
REMARKS: 03:45 am, August 02, 2013. It appears in the investigation conducted by the
officer-in-case (PO1 Magtrayo, RA), that the driver of vehicle suddenly lost control causing
him to crash into an electric post.
NOTE:
Gutierrez and his body guard were advised to undergo medical check-up, while
his assistant was sent to morgue and immediately informed the family.
ACTION TAKEN:
Gutierrez was charged with reckless imprudence resulting to physical
injury and homicide.
This certification is being issued upon request of the party concern for whatever legal
purposes this may serve best.

Theft

(Theft Incident) At about 08:00 am of September 16, 2014, a certain PO2


Magtrayo received a call from PO3 Casas that there was an incident of theft
happened in Seaside, Matina Aplaya, Davao City. At about 12:00 am of September
16, 2014, Ms. Ina Bigatlon, 30 years old and a resident of Seaside, Matina Aplaya,
Davao City. While sleeping inside their house, she notice that there is a person
inside, and she was surprise when she saw a man running and immediately went
down to her bed and chase the alleged suspect. When she was already outside of
their house, she saw their television left outside their house.

3. To determine the surrounding circumstances of the killing of PO1
Armateo
4. To possibly identify the suspect of the killing of PO1 Armateo that will
lead to the apprehension and possible prosecution.

III.

FACTS OF THE CASE


5. At about 02:00am of the 15th March 2009 at Sitio Enrique, Brgy.
Aguinaldo, Bacolod City where in the case of homicide happened
within the above mentioned place. The victim was PO1 Armateo.

6. The said incident transpired at the house of Peter Galloza at sitio


Enrique, Brgy. Aguinaldo, Bacolod City whom also the suspect of the
killing of the said victim.

IV.

DISCUSSION
7. A team led by SPO3 Demie Casas and PO1 Joseph Oclarit
immediately conducted a thorough investigation of the killing of PO1
Armateo. When the team arrived at the crime scene, they saw PO1
Armateo already lying lifeless on the ground under the area of the
Peter Gallozas house.
8. Investigators was able to interviewed all the members of the Galloza
family and the neighborhood present in the crime scene.

V.

CONCLUSIONS
9. After the conduct of an autopsy, a follow-up search for evidence was
immediately conducted at the scene of the crime. During the conduct of
investigation and searching for evidence, the team were able to
determine that the incident was an accidental.
10. The team were able to locate a two .45 pistols with live ammunition
and a gun recoil spring while the .357 revolver was not found during
the investigation. The victim was killed with use of .357 at closed
range.

VI.

RECOMMENDATION
11. Therefor the undersigned suggest that the case like this could have
been prevented if a necessary issue was called attention.
