Individual Support Plans facilitate methodical

agencies can use Theraps service plans and goal
tracking tool to plan, organize and implement
services for each individual in a HIPAA compliant
manner. The ISP module includes program
definition and service data collection which
covers a flexible way of tracking an individuals
progress in achieving their goals. ISP Reports
provide detailed information about individuals
goals and outcomes.

Individual Support Plan


The Individual Support Plan (ISP) provides an enhanced workflow for planning and documenting
supports for an individual. The support plan application captures:

The Individual Support Plan application can be
used to plan, organize and implement support
plans for each individual in an agency. The ISP
module includes program definition, data
collection and report generation features that
offer flexibility in tracking an individuals
progress in achieving their goals.

The ISP module contains details of the supports,


activities, and resources required for the individual to achieve personal goals. Theraps ISP module helps articulate decisions and agreements
made during a person-centered process of planning and information gathering.
The ISP module is designed to be intuitive and to
reflect whats important to and for the individual.
Users can add personal preferences when setting goals, and establishing networks of support.
Major health risks and safety issues in home and
community environments can be assessed and
recorded as well.
In the Therap Multi-Provider system, a Unified
Search option is available for individuals who
receive services from multiple Linked Provider
agencies.

ISP Program
Theraps ISP Program module is designed to facilitate the development of a detailed Individual
Support Plan. It consists of scoring details, tasks
and teaching methods that track progress of an
Individuals specific goals and skills. The ISP Program module is used to design and document
teaching programs that chart the goals or training objectives. An ISP Program for an individual
can be subdivided into one or more tasks. The
tasks assigned can be evaluated by using
various Scoring Methods including Yes/No
responses, the level of independence in
completing tasks, and approaches defined by
users. These scores determine successful
completion of tasks and the level of progress.

ISP Agenda
With the ISP Agenda module users
are able to electronically develop an
agenda of topics to be considered
and discussed in ISP Planning meetings. ISP Agenda gives ISP Team Members an account of the previous years
progress of an individual, providing a
glimpse into factors that worked well
and made sense to implement for the
individual. Development of an Agenda
is a crucial step in the care planning
process. ISP Agenda links to the Individual Support Plan being reviewed as
well as the Personal Focus Worksheet,
which gathers pertinent information

T-Logs are an alternative to your logbooks

and the daily notes that you share with other


staff members at your agency. You may write
T-Logs for specific individuals or for a particular
program or even multiple programs. You can
share these logs with co-workers who have appropriate access privileges. Updates made to
T-Logs can be seen from the Update History.

Types of T-Logs
The T-Logs can be categorized using one or more
of the preset labels Health, Notes, Follow-up,
Behavior, Contacts and General. Types of T-Logs
help staff to organize the notes into the relevant
categories.

Sharing between Programs


T-Logs can be shared between multiple Programs within an agency, for example, a residence and day program can share individuals
information if the individual is enrolled in both
the programs.

Attachments
Two types of external attachments can be added
to T-Logs:
yy External attachments from computer

Tracking T-Logs
T-Logs carry information that need to be seen
and known by everyone supporting an individual in an agency. Thus, Theraps To Do list
on Dashboard helps you to keep track of the
information by displaying a count of T-Logs
that are yet to be read. The Mark as Read
and go to Next feature makes it easy to read
through the large numbers of unread T-Logs.
This will also reduce the count of T-Logs from
your Dashboard, helping you to keep an exact
count of T-Logs that you need to acknowledge.

Notifications
You may select a Notification Level for a T-Log to
signify the importance or urgency of the message. Every user has the ability to configure the
system to receive notifications based on these

levels - High/Medium/Low. You may choose to receive Notifications on your phone as a text message, on your pager, over email or through Theraps Secure Communications (SComm) messages.

maximum size of a single attachment is 3 MB.

The Individual Data module is used to record

the basic demographic, health and emergency information for individuals. All other Therap
forms that are created for them will refer to this
information, making sure the care providers are
well informed. Modules titled Individual Data,
Emergency Data Form (EDF), Individual Home
Page, and Individual Medical Information have
been designed for storing and viewing information regarding different aspects of an individual.

Individual Data Form


The Individual Data (ID) form is designed to help
users record and maintain essential information
regarding the individuals they support. The page
contains basic information such as first and last
names, gender, date of birth, SSN, Medicaid number, admission date, physical features, etc. Users
are able to attach two photographs of the individual and add external files or attach scanned
documents to this module.
If an agency is running programs providing specialized services, an individuals enrollment in
these programs can also be tracked here. The
date of enrollment, physical locations and contact information of these programs are also recorded here.

Medical Information
General medical information regarding the individual is recorded in this section. This includes
blood type, disabilities, adaptive equipment,
emergency orders, primary care physician and
other information, which are available from

other modules for cross reference and as an aid


to decision making and planning.

Diagnoses
Diagnoses and their descriptions can be entered
for an individual. A lookup box helps users search
and pick out the diagnosis from a pre-defined
list. The system provides a comprehensive list of
ICD-9/ICD-10 and DSM IV diagnoses, state based
codes, and other common lists. Another lookup
box helps locate who diagnosed the individual with the condition. The Axis (1/2/3/4/5) and
the diagnosis dates are specified too. Diagnoses
for conditions that have been cured or no
longer exist can be resolved and the details of
when and by whom they were resolved is recorded in the system.

Guidelines
Guidelines and restrictions on diet, mobility,
communication, supervision and other factors
that affect daily living are recorded in this section. These can be written in narrative form and
drop-down menus are available for the ones that
have a known set of options - e.g. for mobility
needs, a user can select from preset options of
Walks on own, Walks with assistance, Uses a
cane, Uses a walker, Wheelchair or Other; and
still enter a descriptive comment.
Other information such as Insurance, Assessment
Score, Team Members and Contacts are also
recorded on this form. Agencies can include data
of their choosing by the use of Custom Fields.
Data entered in Custom Fields can also be included in an EDF and other custom reports.

Emergency Data Forms
The Emergency Data Forms (EDFs) display a
set of information that may be essential in case
of emergencies. Data is automatically extracted from an approved Individual Data form and
shown in a read only view. Users are also given
notifications of emergency information that
have changed. On these forms, a Read and Acknowledge count is shown to the user on their
Dashboard so they can follow up. It can be printed out or exported to PDF format. The EDFs usage also extends to temporary staff who need to
get an overview of the Individuals.

Emergency Contacts
Contacts labeled as Emergency Contact in an
Individual Data form will show up on the

Emergency Data Form (EDF) with available


details including provider agencys contact
information.

Medical Contacts
The EDF displays contact information of an individuals physician, hospital, pharmacy, and
dentist, to be used for medical needs and
emergencies.

Other Data
The EDF shows other data entered into Individual Data including the individuals identification
data, insurance plans, medical data, behavior information and guidelines for attending to the individuals needs. With proper authorization, users
will be able to view an EDF and track whether the
latest EDF has been printed.

Demographic Report
Each agency can generate a demographic report, which
is based on the information entered in the Individual
Data forms. The report displays all the relevant demographic statistics by active/inactive individual, gender,
age, mobility, communication modality, top 10 diagnoses and others. The report also shows counts of individuals enrolled in different programs and supervision
schemes.

Export to Excel
Users have the option to export the Demographic Report
results to an Excel file by clicking on the Export to Excel
link at the bottom of the Demographic Report page.

health data and create detailed and monthly reports. It provides a flexible way to record and follow up on medical issues such as:

alert the relevant teams and/or decision makers.

Blood Glucose
The Blood Glucose module makes it possible for users to record and keep track of the
blood glucose level of an individual in detail,
including the method of measurement and
the administration of insulin. These records
for an individual can be summarized into periodic reports and easily exported to Microsoft Excel to effectively analyze the trends.

Lab Test

The Height/Weight module gives users a simple


and intuitive interface to record the height and
weight of the individuals being supported. The
collected data can be viewed on a graph, table,
or exported to an Excel spreadsheet, for further
analysis.

Vital Signs
This form contains sections for recording an individuals pulse, temperature, respiration, and
blood pressure. The person entering the data
can view a reference table where the normal
ranges for each specific vital sign is displayed.
These normal ranges adhere to the requirements of CCHIT (mandated criteria for LTPAC
EHRs; see more on page 23). The system also
detects an out of range value being entered,

Health Tracking modules have a Search feature


that allows users to quickly reopen an old record.
When one enters the first three letters of an individuals first or last name, the system will pull down
a list of names that match to make it easier to find
and select an individual. Records can also be filtered
by their status (whether In Prep, or draft status; Approved or approved by an authorized colleague; or
Deleted) or their Notification Level (High, Medium or
Low - which signify the records priority).

Intake and Elimination
The Intake/Elimination form has a highly
configurable and user friendly input screen
for saving multiple data entries during the
day. This module makes it easier for the users to track and record fluids, calories, food
intake along with bowel movements, and
other information efficiently.

The Dashboard displays the appointments for
the current day and the next to remind users of
upcoming appointments of individuals.

Appointments
The Appointments module enables users to enter details of and track upcoming appointments
for individuals. Contact Information of physicians, specialists and hospitals are available from
pre-populated lists. Users can specify reason for
appointments and record the follow-up information such as the diagnoses, medication changes,
and lab results. Follow-up appointments can also
be set.
Consultation Forms can also be generated from
Appointments that include information useful
at appointments such as current medications,
Medicaid & Medicare numbers and medical diagnoses.

Medication History
The Medication History module is an efficient
tool for users to keep record of medications given
to individuals. Integrated with First Databank, a
leading drug database, Medication History forms
are equipped with drug lookup tools that also
provide details such as what the drug looks like
and side effects. These measures make sure the
staff have information to avoid medication errors.

Seizure Reports enable users to record and


view the duration of a seizure, the date and
time, and other relevant information, including what actions were taken by staff, what
the precipitating factors were, a description
of the event, and also a comments box to
capture other information.

Health Care Reports


A Health Care Report can be generated for a given time/date range, with summaries from an
individuals data. These may include information
extracted from Health Tracking forms, along with
key information such as allergies, dietary guidelines, and the medical diagnoses. A summary of
the General Events Reports (Module for incident
reports and medication error reports; more on
page 16) created within this date range are also
shown in the report. Once a report is generated, users can save the report within the system.
Comments may also be added to this report by
any user having appropriate privileges to the
individual. Options to generate detailed and
monthly health care reports are also available.

Lab Test Result


The Lab Test Result module gives users the ability to create new lab tests, keep track of scheduled lab tests, and record results of lab tests
for an individual. Therap provides an option to
generate summary reports based on lab test results and tools to follow up on them.

Theraps flexible access control and notifications


system make sure that people involved in providing support to an individual are informed of his or
her current status. Various communication mechanisms allow users to enable notifications for a specific set of actions and levels of concern (e.g. high,
medium or low). Users can also select the appropriate media such as email, cell phones and pager.

What is SComm?

Providing Access

SComm messages are similar to secure emails


that facilitates the exchange of information
among users in an agency, in a secure, HIPAA
compliant way. SComm messages help improve
communication related to services as well as the
operations of the company, e.g. agency wide
meeting announcements.

SComm allows one to select multiple recipients


based on recipient names, titles and program
names. Users can also create fixed mailing lists,
called Custom User Groups. Upon request, Therap allows the exchange of SComm messages between multiple agencies who provide support
to the same individuals. Communication with
Therap Customer Support is possible using the
SComm module. With special roles, users in Oversight Agencies can send SComm messages within that agency and to other providers linked to
that agency. The provision of this role in SComm
allows a user in an agency to share relevant information to oversight organizations from within
Therap.

Sending and Tracking Messages


When sending messages, users get different
options including type and notification levels to categorize the messages. These options
help senders and receivers to filter or search for
messages. Users who have marked messages
as Read are listed in the Readers List of each
SComm message.

Sharing Data
Users can share both general and individual specific information and attach various files and individual specific plans, forms and reports from other Therap modules through SComm messages.
Sharing forms or reports from most modules, e.g.
an ISP Program or a GER (incident report), with
co-workers, is possible from both the SComm interface and the modules interface.
The forms that are shared also follow Theraps
security protocols and only the recipients with
appropriate access privileges are able to open
these attachments. The entire communication
is processed using Theraps secure, HIPAA compliant servers. As a result, the information that is
being shared remains safe and secure.

Notification
SComm messages can be prioritized with High,
Medium and Low notification levels. Users may
choose to receive notifications for SComm messages on a phone and/or pager by configuring
the notification level that the message needs to
have for the user to receive notifications via their
chosen media.

Security
The forms that are sent via SComm also follow
Theraps security protocols and only the recipients with appropriate access privileges are able
to open the form attachments. Communication
through SComm is processed using Theraps secure, HIPAA compliant servers and information
shared remains safe and secure.

The General Event Reports (GER) tool will help

you record incidents comprehensively, with


dropdown menus of options to choose details
from as well as the ability to record witness details and accounts. Events can also be prioritized
using the High, Medium and Low labels to ensure proper follow-up by support professionals.

Record Multiple Incidents in


a Single Report
When multiple events, for instance, an injury and
the use of restraints, need to be recorded on the
same incident report, it can be done with the GER
module.
Each event type includes the option to enter detailed information specific to the event.

Injury
Detailed information regarding the cause of injury, type of injury, severity of the event, treatment
details among various other options can be added. To allow more flexibility users are provided
with a body diagram, a visual tool where service
providers can select body parts that sustained
injuries in the incident.

Restraints and Restraint Related


to Behavior
The GER module allows users to add information
on events where physical, chemical, mechanical,
and other types of restraints were administered.
Summary notes on restraints may be added as well.
Detailed information on restraint related to behavior, including time and date, status and summary can be recorded using GERs. This section is
integrated with Theraps Behavior Tracking module to facilitate reporting or generating clinical
reports.

Recording Abuse & Neglect


A key feature of Theraps GER module is the ability to flag incidents for which abuse or neglect
allegations are suspected. This allows agencies
to track whether such allegations involve actual
physical, psychological or sexual abuse or negligence. Therap ensures automatic and real-time
alerts of incidents and allegations of abuse and
neglect via Secure Communications (Theraps
internal, HIPAA compliant messaging service),
email and text messages.

Medication Error

Approve, Review and Follow-up


Counts

Incidents caused due to a medication error can


be documented here. This GER event contains
fields for medication error type, error details (e.g.,
medication ordered and medication given), prescriber notification and reason for errors among
other options.

Submitted GER forms can be tracked by support


staff for approval, review, and follow-up. Theraps
To Do section of the applications Dashboard
displays a list of counts for new/unread event reports pending review and/or approval. This view
varies depending on access privileges.

Alerts and Notifications


GERs flexible notification mechanism allows
users to enable notifications for a specific set of
actions (e.g. submission, reviewing or approval), and a specific level of notification (e.g. high,
medium or low). Users can also select the appropriate media (Email, cell-phones, pager, etc.) via
which they will receive these alerts.

Comply with State Regulations


GER forms comply with State requirements for
incident reporting enabling providers to comply with State regulations. Therap includes state
specific GER forms, which have been designed
according to the policy of different states. Users
are able to take printable version of the state
specific information regarding an incident. The
forms for each particular state are visible only to
users in agencies of these states. The following is

The GER module provides a robust and ef-

fective solution for documenting, reporting and tracking subsequent investigations


and results regarding incidents of an Individual receiving services. The electronic
workflow of the application increases efficiency by simplifying the processes of documentation, reviewing, approval and oversight.
Users can submit, review, approve, follow-up,
view, and delete GERs based on their assigned access privileges. Once a GER form is
entered, it is automatically made available to

authorized users for reviewing and approval. This ensures that the GER can be properly
reviewed by all involved parties. Relevant
comments can be added to the report as
needed. If a report is found to be incomplete
or incorrect, it can be sent back to the submitter for further work. This ensures integrity
of the information recorded on the GER form.
Depending upon the actions performed, the
status of the GER gets updated. This helps

Flexible Scheduling
Medications can be scheduled automatically and
manually on the MAR form. Users can highlight
respective cells on the grid, to understand when
medications are due to be given to Individuals.
To schedule medications, you can select from
items under a category and a subcategory, each
of which has an extensive list you can choose
from. You may also specify the dose, frequency,
starting date and time of the medication. The
Medication Schedules drop down area has a set
of options such as Everyday, Every Other Day,
or even more specific instructions, such as Every
Monday, or Every Other Tuesday. While scheduling, you can also indicate whether a medication
was Prescribed or Over the Counter. The scheduled medications appear on grids where the person administering the medications can see the
time slots along with the due medications and
administer it accordingly.

is available in read only format if data has not
been recorded on the respective MAR form. To
increase flexibility, we have included the ability
to edit the End Date and add attachments for
approved Medication History forms in case medications need to be administered for an extended
duration.

Filters and Jumps


Considering that an MAR for an individual can
have multiple medications and appear very long,
Therap allows users to filter the medications for
Individuals by the medication types (e.g. PRN
Medication or PRN Treatment), the program from
where it was scheduled, or a time period during which a medication is to be administered.
A Jump to menu lists all the medications on an
MAR and with one click, you will be taken to the

HITSP C32 is a summary document that uses HL7 CCD specification to demonstrate the ability
to file and display individuals summary documents. Additionally, the requirement applies to
generate a valid HITSP C32 document using the required terminologies and the ability to display
HITSP C32/CCD documents and file them as intact documents within the EHR. HITSP C32 Documents using HL7 CCD component (V2.5) can also be generated and imported from Therap using
the Health Information Exchange section located at the Individual Data module.

Password Reset
With the Therap for Android app, agency administrators can reset passwords of staff
members from their mobile devices. It is especially helpful for agencies in a distributed setting where overnight staff may forget their passwords. Therap recommends
setting strong passwords of mixed case alphanumeric characters and symbols. Implementing this basic security measure helps agencies make sure access to their
data is protected and security is not compromised.
This tool enables Case Managers, Supervisors, and other authorized users to change
passwords for other users in their agency without having to login to their computers.
The interface shows a list of users in the agency from which authorized users can
change passwords temporarily simply by selecting a particular user from the agency
user list. For security purposes, users with new or reset passwords need to change
their passwords upon first login.

The Personal Finance module allows users to

record information such as transaction category,


date, amount, check/receipt numbers, etc. They
can also attach scanned or electronic copies of
transaction receipts for each transaction. Users
can generate periodic transaction reports for all
or some accounts using a wide range of parameters including transaction period, the date the
transaction was reported, whether the transaction was a deposit or expense, account type,
account status, etc.

For each individual, users can generate a report


that includes all their accounts or just the ones
they select. The report can be generated for
particular types of transactions as well, such as
expenses on food and entertainment, or cash
deposits or deposits from work, etc. Sorting the
report by the institutions and the merchant
where the transaction occurred allow more flexibility in how the reports are generated. Such
reports can show periodic trends in the money
management of an individual.

Training sessions can be scheduled and instructors can be assigned to them. When users sign up

TMS module enables users to add instructors for
training class, create a list of training instructor,

weekly schedules with one or more time slots

time slot. These weekly schedules can be shared
across different programs within the agency.
The Staff Scheduling module allows users to set
up schedules for various types of support professionals ranging from a staff member in a residential program whose work pattern is fairly similar
every week to a specialist working across multiple
programs whose schedule varies widely from one
week to the next. Staff members in a program can
view their own schedules as well as their co-workers on their calendars. The module allows users
to see their schedule across programs as well.

Templates may be created for weekly schedules that remain unchanged over one or
two weeks. If the weekly schedules vary
widely on a weekly basis, the system provides flexibility to include these variations in
weekly schedules as per agency requirements.

Therap Calendar
Therap Calendar enables staff to view their
own schedules from their accounts. It further facilitates better planning and management of different events in a program.
The calendar allows users to track and view
appointment schedules, save day notes,
mark dates for special staffing needs and
other daily events. The calendar displays
monthly, weekly and daily views of the calendar entries. This feature supports exporting to PDF for users to print calendar pages.
