ENROLMENT PROCEDURE

For CROSS- ENROLEES from OTHER SCHOOLS


EFFECTIVE SUMMER 2009

FLOW DIAGRAM DETAILS

Registrar 1. Seek approval from the Registrar for cross-enrolment.

SAO 2. Go to the Student Affairs Office (SAO) for the following:


a. Submission of requirements
♦ Original copy of Certification of Good Moral Character
♦ Permit to cross- enroll
♦ Two (2) pcs. 1”x1” black and white ID pictures
b. Interview and approval of Enrolment Admission by the
SAO Head
c. Issuance of Student ID Number

OAS 3. Go to the Office of Admissions and Scholarships (OAS)


for approval of Enrolment Admission and picture taking

College/ 4. Go to the College/ Department for the following:


Departme a. Approval of your Enrolment Admission by the Dean/
nt Department Chair
b. Encoding of your tentative subject- load which will serve as
your reservation for 24 hours
c. Printing/ issuance of the duly signed computer-generated
enrolment form.

Accountin 5. Go to the Accounting Department for payment


g ♦ For students who will be paying in full, the Accounting
Department will do the evaluation of fees

ETO 6. Proceed to the Enrolment Technical Office (ETO):


a. Submit the remaining copies of your computer-
generated enrolment form together with the required
documents
b. Present your official receipt of payment for the
evaluation of fees and printing of temporary study
load.

Registrar’ 7. Go to the Registrar’s Office on the date specified in the


s temporary study load and claim your official study load and ID card.
Failure to claim would mean “no official enrolment”.

IMPORTANT REMINDERS :
 Keep your official study load and ID card as these will be inspected by
your teachers during the FIRST WEEK of classes.
 If your ID card is lost, see the SAO immediately.
 If your official study load is lost, see your Records In-Charge at the
Registrar’s Office.
 Present clearance and surrender your ID card upon claiming your final
grade at the Registrar’s Office.
