REAL-LIFE QUESTIONS
Given my education, what are my options
in psychiatric nursing?
Nursing school has exposed me to many wonderful opportunities to explore my
interests. I recently completed an internship for a senior adult day program, and Ive
also worked for a local mental health clinic. From these experiences, as well as my
classes, Ive discovered that I love psychiatric nursing. I found out that I prefer to work

with people who suffer from pathological disorders rather than behavioral problems. I
also liked the senior population because they are very appreciative, and you can see
that they benefit so much from the attention and therapy they receive.
Even though I have a strong sense of what I like to do, Im not sure what direction
to take next. Throughout my clinicals, Ive felt a little unsure about the setting I want to
work in. The work environments in the smaller, local clinics dont appeal to me because
most of their patients seem to exhibit conduct and substance-related disorders rather
than clinical illnesses such as schizophrenia, which is my main interest. Ive also heard
that theres a high degree of burnout among staff. I dont think I want to work in
hospitals, either, but I wonder if that will hinder my nursing career.
Presently Im working part-time as a pharmacy technician for a mental health clinic,
and Im enjoying it. Many people tell me that the field of pharmaceutical sales is wide
open and is hiring BSNs right out of college, but I fear that such a position may take me
away from patient contact. Because Im a senior and will be graduating soon, Im hoping
to find my niche. Given my education, what are my options in psychiatric nursing?

In her book From Novice to Expert (1984), Patricia Benner describes the transitions
nurses make from one level of knowledge and competency to another. She says that
the gift of being a novice is that it offers a tremendous opportunity for growth and
professional development. The challenge is to discover, often by trial and error,
strategies that develop the dimensions of psychiatric nursing needed for expertise. It is
vital to read professional journals, attend seminars, and join and volunteer in
professional psychiatric nursing and mental health organizations to network and form
mentor relationships. These relationships can provide invaluable input about this
multifaceted field.
In setting realistic goals as a new graduate, I recommend working for at least one
year as a community mental health nurse. This can be instrumental in understanding
the system all the way from payer sources to direct client services, referral resources,
and creating partnerships and connections. Every mental illness has physiological and
behavioral components. Schizophrenic clients may have substance abuse problems as
well. Hospital experience is difficult to replace. It is in this setting that the schizophrenic
client resides when decompensation occurs. In either inpatient or outpatient settings,
the psychiatric nurse is the best therapeutic tool for modeling healthy behavior. It is also
important to check with your State Nurse Practice Act to see what standards and parameters for practice a bachelor of science registered nurse degree allows.

With breadth of knowledge and experience, psychiatric nurses can serve as client
advocates in a special way. Social activism on behalf of this vulnerable, high-risk population is a truly important societal need. Nurses grounded in holistic health, the integration of body, mind, and spirit, can expand and evolve models of psychiatric care.
Graduate school preparation will further increase your capacity to develop knowledge,
expertise, and credibility that can be translated into action. The importance of education
and training throughout ones professional career cannot be stressed enough.
Competence in nursing develops over time. The key to prevention of burnout is setting limits and maintaining principles of self-care. And remember, when you feel overwhelmed and discouraged, staying grounded in your purpose and passion for becoming
a nurse will help you endure these growth-producing times. To determine what you
want to be, it is important to know who you want to become. Writing a personal mission
statement will help you reflect on and define your deepest values, talents, and sense of
mission needed to guide your professional development. To stimulate thinking along
these lines, I highly recommend Barbara M. Dosseys book Florence Nightingale: Mystic,
Visionary, Healer (Lippincott, Williams & Wilkins, 2000). Our challenge is to continue to
grow and take initiative to stimulate positive change in our own lives, as well as work
with integrity in the health care institutions we serve. The proud history of nursing provides strength for the journey.

The American Association of Colleges of Nursing (AACN) adds the following to the list of supply problems:
Job burnout and dissatisfaction. Nurses are leaving the profession as a
result of working conditions that lead to burnout and job dissatisfaction. The AACN reports on a study by Linda Aiken in the October
2002 issue of the Journal of the American Medical Association that
nurses reported greater job dissatisfaction and emotional exhaustion
when they were responsible for more patients than they can safely care
for. Researcher Aiken concluded that failure to retain nurses contributes to avoidable patient deaths.9
The shortage is affecting patient access to health care.10 Patients are
more prone to complications and other problems when there are not
enough nurses. Numerous reports and studies have linked nursing care
with patient safety and good outcomes. Without nurses, patients and
health care suffer.

would improve the shortage.
Nursing offers many career options, which is a plus in terms of employment opportunity, scholarships and grants to attend school, and encouragement from nursing organizations. At the same time, as a professional nurse,
you will be faced with many challenges and difficulties in trying to meet the
needs of society. But take heart: Opportunities abound, options are abundant, and a life of meaningful work is within your grasp.

Caring
What does caring mean to you? Caring is a term that is so strongly
associated with nursing, it is usually the first thing people think about being
a nurse. Care is the heart and soul of nursing. Caring means taking the time
for active listening; advocating for those in need; valuing and respecting all
individuals; being able to examine your own biases and reflect on your
thoughts and actions; and such things as making pain relief a priority and
the healing process an act of body, mind, and spirit.
Compassion
Compassion is the ability to be considerate, humane, merciful, and kind.
Being considerate of anothers needs despite your own beliefs, acting with
respect and concern for anothers well-being, and being able to assist
others toward a full development of their potential in any given
circumstance is what compassion is all about.
Knowledge
Understanding theories used in nursing, including scientific theories, those
related to health behaviors, and especially nursing theories and diagnoses,
is essential. Knowledge includes the ability to put theory and research into
practice using the types of critical thinking and inquiry skills as discussed
in Chapter 5.
Dedication
Commitment with diligence is essential in nursing practice. Sticking with a
project, despite initial or repeated problems, is especially important in
research, in your studies, and in nursing practice. Attention to detail and
careful analysis and execution of procedures can be developed in science
classes and, later, in nursing school.

Professionalism
Professional behavior is developed through belonging to, supporting, and
participating in nursing organizations. These organizations, such as the
American Nurses Association, work on many levels to promote nursing
practice, health and welfare of the public, and health policy through social
and political action. Professional actions include respect for others at all
times and acting with integrity and for the best of your clients and fellow
nursing colleagues.
Advocacy
To advocate for yourself, another individual, or a community means that
you use your expertise as an RN to protect human rights. RNs may do this
by helping others make informed decisions or by acting as a mediator
between a client and a doctor, family members, or even the legal system. To
advocate is to inform and support a person, provide desired information,
and present information in a way that can be understood. It also means
understanding that someone may not want information. Advocacy means
that an individuals or groups needs are your main concern. It requires you
to be assertive, convey concern, understand different communication styles,
and practice good working relationships.
Creativity
Many discoveries and solutions to problems come from creativity or from a
mind that can see things just a little bit differently from others. Having a
broad education and experience in literature, philosophy, and politics will
help you develop the ability to view problems in fresh new ways. Each field
of study has its ways of viewing and understanding the world. The more of
these you learn, the more flexible and adaptable you will be in your ability
and skills as a nursing student and as a nurse.
Mathematics
Youve no doubt heard people freely admit that they have a problem with
math, or a math block. Have you ever heard anyone freely admit that
they cant read? Admission of this problem is perceived as shameful, yet
saying the same of math is acceptable. What does this say about our view
of math skills? The ability to use math is essential in nursing. Most nursing
schools require you to take a math test of medication calculations before
you can proceed in your course work. You will also need to take a course
in statistics to learn about nursing research. Math skills are not optional in
the information age of the twenty-first century.
Observation
Observation is a skill that many nurses will tell you is their greatest asset as
a nurse. The ability to study people and nature, see patterns, and notice
things that others may not notice requires astute observational skills. All

nursing endeavors demand the ability to see, hear, smell, and touch.
Observation skills can be enhanced in many ways, such as by taking a
course on how to identify plants and trees. Anatomy, microbiology, even
art courses allow you plenty of opportunity to practice observing attributes
of things you may not have noticed before. Observation is a skill that is
developed over time and one that is crucial to the assessment processa
huge part of being a nurse.
Critical Thinking
Do you question what others say? What you read? What you see? If so, you
have what is called healthy skepticism, and healthy skepticism is important
to critical thinking. For instance, suppose you read a newspaper article on
the effect of marijuana use on mental illness. Your understanding of the
article is that there is a link between past use of marijuana and present-day
symptoms of depression. In other words, the article implies that marijuana
use is a cause of mental illness. However, if you think critically you may
question this cause-and-effect statement: Are people with mental illness, or
who have a predisposition to mental illness, more likely to smoke
marijuana in the first place? Is the articles implication right? What else
could cause the problem?
As a nurse you will need to do this kind of thinking. Here is another example: An elderly patient who has had frequent falls and increasing instability when walking comes into the emergency department. She is sent off
for tests to rule out neurological problems and chemical imbalances. You,
the RN, ask the question: What else could cause the symptoms? You examine her shoes and find that one has a missing heel and a large hole worn
through the bottom. After numerous tests at great cost, you discover, by using your critical thinking, that her shoes have been the source of the problem all along. Critical thinking will save lives, save money, and improve
your patients quality of life.

56% percent of that care occurs in hospitals. But the number of RNs
working in other settings is increasing as changes in health care systems

advocacy mentioned earlier in this chapter); giving continuity of care; and
providing care that balances quality, patient needs, cost, and resource use.19
Educational preparation. Most nurses working in ambulatory care have
some experience in nursing and a BSN. The expansion of the role requires
increasing coordination and management in the health care network and
community. One area that is expanding is telehealth (covered below), a new
subspecialty in which nursing care is provided using telephones, computers,
and videos. New graduates may be hired if they have school experience in
ambulatory care settings.

specialty. Some characteristics are common among parish nurses. They are
RNs, are part of the ministry staff, have taken courses in parish nursing,

Get practical about new areas in nursing you havent considered.
Go to one of the nursing websites listed at the end of this chapter. Explore the
sites to find a field of nursing that you had not considered in the past but one
that looks potentially interesting.

As a nursing student, you will need to take your commitment seriously. You will want to consult advisers to discuss such topics as

families. If you plan to apply to other schools for further nursing education, talk to advisers about their admissions criteria so you can plan
ahead. Many schools today are turning students away because of a nursing faculty shortage.
Nursing school is a rewarding experience. It will open you to a whole
new way of seeing the world. It will prepare you for a lifelong career that is
exciting and rewarding. You will be changed forever by attending nursing
school; you will make new friends, learn incredible amounts of knowledge,
change behaviors, and become a professional. Preparing for this experience
is vital. Talk to other nursing students, advisers, and nurses to help you get
ready.

think you are doing all the learning and your instructors all the teaching.
But you teach when helping other students in laboratories and in study
groups, and you teach your instructors through inquiring questions,

Collaborative Solutions
Alertness to Real-World Research in the News. To
be a success in nursing, you must study all aspects of it,
including what is in the news. Pay attention to what
appears in the popular press (newspapers, magazines, television news) and
in scientific journals.
1. For one week, review a local newspaper and the local television news.
Keep notes on the health news presented.
2. In class, divide into small groups and discuss your notes, looking for
common threads between the newsworthy information and other
social or political trends.
3. In small groups, discuss whether there is a great deal of information
about nurses or very little. Discuss why.
Alertness to Nursing Images on TV and in the Movies. A new
campaign is under way by the Baltimore-based Center for Nursing
Advocacy. The center says that medical dramas on television such as Foxs
House and ABCs Greys Anatomy are misleading in showing nurses as
insignificant in patient care. The shows feature physicians and interns
doing things that in real life nurses would be responsible for. Shows
frequently portray nurses as uneducated or subservient. The shows, if
they do portray an intelligent nurse, show that person as leaving nursing
to become a doctor instead of highlighting the advancement opportunities
in nursing. The American Nurses Association is concerned that
misleading television shows will affect young peoples decisions to become
nurses, worsening the shortage. TV producers justify their actions saying
it is for entertainment, not to be realistic.
Nurses on TV or in the movies are fictional characters meant to make
simulations of supposed real-life emergencies realistic and, at least,
dramatic. The purpose of this exercise is to discover images that are
positive and those that are negative. Remember, absence of nurses in
situations in which nurses would normally be present is a negative image. It
makes nursing work invisible and unimportant, or it implies that others do
the work of nurses.

WRITING
Discovery Through Journaling
Record your thoughts on a separate piece of paper or in a journal.
Observing What You Already Observed. Return to your first
journal entry and read it through. Next, return to the initial
observation site and begin observing it again. Record all new
observations in your journal. Spend a minimum of 10 minutes of
continuous, uninterrupted writing. When you are finished, read
what you wrote. Think about how your first and second observations
differed and how they were the same. Reflect on this observation process
and write your thoughts or feelings about it.
CAREER PORTFOLIO
Plan for Success
Interests, Majors, and Careers. On a separate sheet,
write for 5 to 10 minutes on the following, and then
share in small groups: What in your view is the most
important thing that influenced you to think about a
career in nursing? Write a story about that situation or idea. Be as
specific as you can, using sights, smells, feelings, places, times. In the
small group, talk about why you think your story was influential in
your career decision.
