AFFIDAVIT OF LOSS
I, CEFERINA MORALA, Filipino citizen, of legal age, married, and a
resident of Davao City, after having been duly sworn in accordance with law,
hereby depose and say that:
1. I am the Secretary of ALAY SA FIL-MISSION FOUNDATION, INC.;
2. The Bureau of Internal Revenue has issued a Certificate of
Registration with TIN 004-724-840-001 to ALAY SA FIL-MISSION
FOUNDATION, INC which I kept in my possession for safekeeping;
3. Sometime in June 2015, I discovered that the Certificate of
Registration of ALAY SA FIL-MISSION FOUNDATION, INC with TIN
004-724-840-001 which was in my possession was missing;
4. immediately upon discovery that the Certificate of Registration was
missing, diligent efforts were made to search for it but the same
could not be found;
5. that I am executing this affidavit to apprise the authorities on the
true facts of the circumstances in connection with the loss of the
Certificate of Registration of ALAY SA FIL-MISSION FOUNDATION,
INC. and for whatever legal purpose this may serve.
