The Baby Friendly Hospital Initiative (BFHI) is in an effort to increase breastfeeding rates and, in turn, reduce childhood illness and death. BFHI
includes practices that are proven to promote, protect, and support breastfeeding. A grant from First 5 LA established in Oct. 2011 and ending Dec.
2014 supported the hospitals effort to achieve Baby-Friendly designation, which it did in October 2014. The hospital retains designation for a period
of five years, during which there is an annual fee of $1,250.
Bet Tzedek - Legal Support Services are provided in partnership with the non-profit organization Bet Tzedek at no cost to Health Benefits Resource
Services clients who need assistance and/or advice regarding issues such as Landlord/Tenant Law, Kinship Care, and Power of Attorney for Health
Care. The Legal Support Services program at St. Francis Medical Center will be concluding on July 31, 2015, as similar resources are available through
the Legal Aid Foundation, Bet Tzedek locations throughout the LA area (Wilshire district office and senior centers within the region), and 2-1-1 Health
and Social Service Referral Line. SFMC cost to closure June & July 2015: $17,479.
Casa de Esperanza, collaborating with Catholic Charities of Long Beach, provides temporary housing and support services to homeless families and
helps them transition to self-sufficiency. The program ended 06/30/14 as the building is in the process of being sold. SFMC program cost: $0 cost
remain.
Childrens Counseling Center School-Based Counselor provides counseling and educational services on the campuses of four Daughters of Charity
schools within the South Los Angeles area. The school-based counselor addresses emotional and behavioral distress, and helps reduce a significant

barrier to successful learning. The counselor is fully funded through a grant from the Daughters of Charity Foundation, which has committed grantfunding of the program through FY16 (June 30, 2016). SFMC program cost: $0.
South Los Angeles Access to Care SFMC Compton Clinic provides preventive and primary care, including lab work, to Compton residents who lack
regular access to health care. The Los Angeles County grant ends 06/30/15, and the program will conclude at that time. Estimated cost to closure
$19,000. The grant fully covered all program costs. SFMC program cost: $0.
COPE Clinical Health Extenders is a program for clinical interns. SFMC plans to maintain the program at its current level. Estimated FY16 SFMC
program cost: $94,400.
Dental Grant pays for dental work of children with special needs. The Jim Mora Count on Me Family Foundation grant fully funded this community
benefit program. The grant for the FY15 period has ended. The program has concluded. SFMC program cost: $0.
Diabetes Education and Disease Management responds directly to the rise in diabetes rates in our Community. This program was integrated into
Healthy Community Initiatives in FY13. SFMC program cost: $0.
Footprints ensures that children have shoes and clothing for school. It provides new footwear and uniforms for students, and new and used clothing,
toiletries, and food items to families. The building out of which Footprints operated was sold in 2014 and the program has concluded. SFMC program
costs: $0.
Health Benefits Resource Center (HBRC) is a one-stop venue providing enrollment assistance, information and referrals for uninsured low-income
residents, helping families gain access to public and private health insurance, social services and nutrition programs. It also educates residents about
the Covered California program. Staffing has been reduced through attrition and services will be offered on a smaller scale in FY16. Historically, HBRC
has been fully grant funded except for some indirect expenses. In FY15, various grants fully are funding the program. In FY16, grants will fund the
majority of the program. Estimated FY16 SFMC program costs: $9,012.
Healthy Community Initiatives (HCI) brings health screenings, immunizations, and health education directly to area schools, churches, businesses and
community organizations via its HCI nurse, educator, staff, and mobile unit. Historically, HCI has been fully grant funded except some indirect
expenses. In FY15 and FY16, grants will fully fund the program. FY16 SFMC program costs: $0.
Indigent Care program provides for reimbursement to SFMC affiliated physicians for the care they provide to indigent patients. It is provided through
county contracts with SFMC and through subsidies from the Daughters of Charity Foundation. In FY15 and FY16, reimbursement through the county
contracts is anticipated to cover costs. FY15 and FY16 SFMC estimated program costs: $0.

Naseau Reading Program is daily help with reading for children reading two years or more below grade level. The building out of which the Naseau
Reading Program operated was sold in 2014, and the program concluded 06/30/14. No cost remaining.
Paramedic Training and Education provided through the hospitals Emergency Department offers a means of training for LA County EMTs, RNs, and
MICNs to enable them to keep licenses current and pre-hospital care consistent. SFMC will continue to offer this education in FY16. Estimated FY16
SFMC cost: $141,000.
Patient Transportation is provided to individuals without any means of transportation for outpatient care and treatment. Service is also arranged for
individuals receiving behavioral health services at the medical center. Scheduled services for outpatient appointments concluded in October 2014.
No costs remain.
Senior Wellness and Education provides health care and wellness education on subjects ranging from diabetes management, stroke symptoms, and
prescription medication management, in addition to free flu immunizations for the elderly. Estimated FY15 SFMC program costs: $22,000. Program
costs in FY16 are anticipated to be the same.
St. Francis Career College (SFCC) prepares students to succeed in health care professions by providing value-based education and career training.
Comprehensive support services as well as skill development sessions help to ensure students achievement and completion of the program. The
program concluded 06/30/13.
Support Groups are offered by our Spiritual Care Services team, including Mothers of Aids Patients (MAP), Gabriel Support (Sudden Infant Death
Syndrome), and Bereavement Support, in addition to post-traumatic spiritual counseling. In FY15, the Cancer Support Group concluded as other
community organizations offered these services so program costs decreased this year from previous year. FY15 SFMC program costs: $62,176. In
FY16, the Post-Traumatic Stress Syndrome (PTSD) counselor will be entering seminary school; therefore, PTSD counseling services will be vastly
reduced. Estimated FY16 SFMC program costs: $12,156.
Trauma and Injury Prevention Program and Southern California Crossroads work with schools, hospitals, and other public and private organizations
to help prevent traumatic injury. SFMC collaborates with the Violence Prevention Coalition, HAVEN (Hospitals Against Violence-Empowering
Neighborhoods, Every 15 Minutes drunk-driving prevention, ICAN (Inter-Agency Council on Child Abuse and Neglect), Walk This Way safe street
crossing, Los Angeles Violence Intervention Training Academy, Urban Peace Academy, and local safety fairs. Trauma and Injury Prevention FY15 SFMC
program costs: $65,000. Program costs are anticipated to be the same in FY16. Southern California Crossroads FY15 SFMC cost to closure (June 2015
cost): $6,143.
Vida Sana/Healthy Life Wellness Program , by working through Southeast Los Angeles area churches, aims at building a healthier community through
a 6-month, coordinated program which provides heart health, diabetes, and obesity awareness, prevention, and education; diet and nutrition classes;
stress reduction; and physical fitness activities to establish healthy lifestyle habits. Vida Sana was integrated with the Healthy Community Initiatives
program in FY14. No program costs.

provide services to the poor and underserved. The program concluded in FY14. FY15 SFMC program costs: $0.
Welcome Baby provides pre-natal, hospital care and post-partum services to low income mothers. The program is fully grant funded through First 5
LA. In FY15 and FY16, SFMC program costs: $0.
