Space available on a first come first served basis. We limit the number of food
vendors and there will be no duplication of food types. Along with your application,
you must submit a proposed itemized list of menu offerings.
Gates open on Friday, October 21, 2016 at 4:00 pm for all vendor booth(s) set up.
Gates open on Saturday, October 22, 2016 at 7:00 am for all vendor booth(s) set
up.
Any necessary items for booth set up, including tables and chairs are the
responsibility of the vendor.
2016 VENDOR REGISTRATION
LIABILITY: In consideration of the acceptance of the right to participate, the
vendor, by execution of this entry form, releases and discharges.
SET UP: Please check in at the registration booth on site for your location*.
Electricity and water will be available. BREAK DOWN AND CLEAN UP: Each
vendor is required to break down their booth after the close of the event. Booths,
tents, displays, trash etc. Clean-up of booths and surrounding areas is the
responsibility of the vendor. All vendors are responsible for their own equipment
that is left on site Friday night.
The Word International Worship Center & Music Festival 2016, City of Hazlehurst and their
officers, directors, trustees, employees, agents, representatives, volunteers, and servants,
and anyone else connected with the management or presentation of The Music Festival
2016, of and from all known or unknown damages, injuries, losses, judgments, and/or
claims, from any cause whatsoever that may be suffered by vendor to his person or
property. Further, each vendor expressly agrees to indemnify all of the forgoing entities,
firms, persons, and bodies of and from any and all liability occasioned or resulting from the
conduct of vendors or any participants assisting or cooperating with vendors and under the
direction or control of vendors.

General Release and Acceptance of Rules: This application covers any and all festivals listed
to which vendor applies or is accepted. I/We have read the Conditions of the Festivals
enclosed with this application. I/We give permission to use my name, business name, images
taken of me or my items for any and all purposes.
This is an ALCOHOL FREE event. Absolutely NO sales or personal use of alcohol
allowed.

Vendor realizes that this is a rain or shine event and no refunds will be
granted due to weather.

I have read this form and agree to all conditions of The Word International
Worship Center/Hazlehurst Music Festival application and to rules governing the
event as set forth on this form and agree to observe all the rules and directions of
the event management.
