APPRENTICESHIP AGREEMENT
KNOW ALL MEN BY THESE PRESENTS:
This contract made and executed by and between:
____________________________________, a domestic corporation duly
organized and existing under Philippines laws with business address at
____________________, Philippines, represented by its Manager,
___________________________, of legal age, Filipino, single and with
address at ______________________________ hereafter referred to as the
EMPLOYER;
-and__________________________, of legal age, Filipino, single and with
permanent address at _________________________________, hereafter
referred to as the APPRENTICE;
-WITNESSETHWHEREAS, the occupation of ________________________ is included in the
List of Officially Recognized Apprenticeable Occupations in the Office of
Apprenticeship;
WHEREAS, Apprentice ____________________ is of legal age having been
born on _________________________ and agrees to train as ______________________
under an Apprenticeship Agreement;
WHEREAS, the EMPLOYER and APPRENTICE agree on the following terms and
conditions:
1. TERM :
The term of this Apprenticeship Agreement shall commence on
___________________ and will end on __________________;
The aforesaid period is considered a probationary period of the
apprentice;
3. PLACE OF WORK and TRAINING:
The Apprentice shall train as _____________________ at
___________________ located at __________________________ and will be
under the TRAINING of the MANAGER;

4. TIME OF TRAINING: Apprentice agrees to work 8AM-5PM with 1 hour break from
Monday - Saturday.
Employer reserves the right to require the employee to train on different days and/or
different schedule to meet the increase or decrease in demand for the products and
services of the pharmacy;

The Apprentice shall approximately undergo 424 hours of on-the-job training as well as
supplementary theoretical instruction during her training.
A schedule of the work processes of the trade/occupation in which the apprentice shall be
trained and the approximate time to be spent on the job in each process is attached hereto
to form part of this agreement.
5. COMPENSATION AND OTHER BENEFITS: That under existing laws, Apprentice
shall be entitled to at least 75% of the minimum wage, thus,
a. Apprentice shall be given Php ____________ per day
which shall be given every 16th and 1st day of the month;
b. The No Work No Pay Policy shall apply.
6. ADHERENCE TO POLICIES
The APPRENTICE undertakes to follow strictly all existing
policies and code of discipline of the EMPLOYER;
7. PERFORMANCE STANDARD FOR REGULARIZATION
The APPRENTICE undertakes to pass the following standards for

documents/papers/certificates required by the Employer

9. FAILURE TO PASS THE STANDARDS FOR REGULARIZATION


The APPRENTICEs performance shall be assessed as determined
by the employer. APPRENTICE shall be required to undergo written and
oral examination relating to the products being sold by the employer
including marketing and customer relations questions. The examination
may be given by the employer or the Franchisor.
In case of failure of the APPRENTICE to pass any of the
standards mentioned in paragraph 8 and/or failure to pass the written and
oral examination, the EMPLOYER shall have the right to terminate this
agreement even before the expiration of this contract.
8. RESIGNATION

In case of resignation by the APPRENTICE, she is required to


notify the EMPLOYER in writing, of her intention to resign at least
THIRTY (30) days prior to the intended date of resignation. Failure to
comply with the 30-day notice requirement shall entitle the EMPLOYER to
liquidated damages in the amount of not less than Php 50,000.00;
10. TERMINATION
The EMPLOYER reserves the right to terminate this agreement
under the following conditions:
a) For just causes and authorized causes under the provisions
of the Labor Code and
b) For failure to comply with the existing policies and code of
discipline of the EMPLOYER;
c) For failure to pass the requirements for standardization
and/or failure to pass the written or oral examination given by
employer or its Franchisor.
11. SERVICE OF NOTICE:
All written notices to the parties shall preferably be by personal
service but in the event that personal service cannot be complied with or
there is refusal to accept the written notice, such notice shall be sent by
registered mail to the address of the parties indicated in this contract
unless a written notice of change of address has been served by the parties.
The notice sent by registered mail to said address shall be sufficient and is
deemed the actual notice to the parties within 3 days from mailing thereof.
IN WITNESS WHEREOF, we have signed this document at _________ City,
