CommunityHealthWorkerLabstudentsmeeteverytwoweeksduringthesemesterfora
seminarwiththeinstructor,andspendaminimumof8hoursperweek(130hourstotal)in
supervisedoutreachtasksinaSonomaCountyhealthrelatedagency,organization,healthcenter
orprogramandthecommunityitservespracticingskillslearnedinCHW151,Outreach
TechniquesandStrategies.
StudentLearningOutcomesforthisCourse:
Uponcompletionofthiscourse,studentswillbeableto:
1. ApplyCHWstandardsofpracticetocommunityhealthoutreachexperience.
2. DemonstrateCHWoutreachtechniquesandstrategiesinhealthrelatedworkwith
communitybasedagencies,clinics,andotherlocalorganizations.
3. Motivatehealthybehaviorchangeinindividualsinacommunitysetting.
4. Demonstratetheabilitytoreferclientstoappropriateservicesandresources.
CourseRequirements,Grading,andExpectations:
AttendanceandParticipation
Thecampuspolicyonattendanceisthatstudentsareexpectedtoattendallsessionsofacourse.
Studentscanbedroppedif10%ofthecourseismissedoriftheymissthefirst2sessionsofthe
course.Itisveryimportanttobeontimeandtokeepallinternshipobligations,unlessyouareill
oranemergencyhasoccurred.
Ifyoumustmissinternshiptime,contactyourinternshipsupervisorandinstructorbeforethe
timeyouweretoattend.Contactyourinstructorifyoucannotattendoneoftheseminars.

GeneralInternshipConduct:
Allstudentswillbeexpectedtoconductthemselvesinamannerthatisrespectful,courteousand
professionaltoclients,supervisors,agencycoworkers,fellowstudents,andtheinstructor.
InternshipHoursandHomeworkAssignments
Hoursandtaskscompletedwillbedocumentedandsignedbyyoursupervisors.Formsanda
timesheet,plusweeklyreportwillhelpwiththisongoingdocumentation.
Emailmeyourcompletedassignments
Homeworkwillincludereflectionsandassignmentsrelatedtoyouroutreachexperience.Points
willbelostforlatehomework.Itisexpectedthatyouwilldoyourownworkonassignments
(unlessotherwiseindicated)andonyourfinalpaper.Workthatisnotyourownwillresultinan
Fgrade.
MeetingswithSupervisor,Evaluations,andFinalPaper
Youwillmeetwithyourinternshipsupervisorfrequentlyduringthesemester,andyour
instructorwillmeetwithandcontactyoursupervisorseveraltimesduringthesemester,
dependingonthetypeofworkyouaredoing,needforclarifications,etc.Thefinalmeetingwill
includeareviewofaccomplishmentsandevaluationofyourinternshipexperience.
Yourcommunitysupervisorwillcompleteanevaluationformanddiscusshowwellyouhave
masteredtasksduringyourinternship.Yourinstructorwillalsoevaluateyourparticipation,
professionalism,accuracy,timeliness,andcompletionofassignedinternshipworkandclass
work.
A23pagepaperaboutyourcommunityhealthworkerinternshipexperienceandbrief
presentationinclasswillserveasafinalexamination.
