Three Priority Diagnoses:


1. Inability to maintain the normal level of blood sugar related to Deficient knowledge about the disease and effects of
complicated Diabetes Mellitus
2. Inability to manage the complications brought about by uncontrolled diabetes mellitus type II related to Deficient
knowledge of the disease process depicted by continued practice of superstitious beliefs and albularyo
3. Inability to maintain the normal level of blood sugar related to Lack of initiative to engage in maintenance of a low
cholesterol, low carbohydrate and high fiber diet and regular exercise
Other nursing problems/diagnoses
4. Inability to maintain the normal level of blood sugar related to inadequate resources to buy the maintenance
medications needed to control Diabetes Mellitus type II
5. Inability to manage the complications brought about by uncontrolled diabetes mellitus type II related to
inadequate resources to control diabetes mellitus II by failing to adhere to pharmaceutical regimens intended for DM type II
6. Inability to maintain the normal level of blood sugar related to Lack of knowledge about available resources for blood
sugar testing at the Health Center
7. Inability to maintain the normal level of blood sugar related to Lack of monetary sources from the government for the
hospitalizations needed for the uncontrolled cases of Diabetes Mellitus
8. Inability to manage the complications brought about by uncontrolled diabetes mellitus type II related to
inadequate knowledge of the effect of diabetes mellitus II to other important organs of the body such as the heart, brain and
kidneys

Community Data:
30% cases of uncontrolled diabetes mellitus type 2 as a health deficit in Banawe City
Community Diagnosis:
Inability to maintain the normal level of blood sugar related to Deficient knowledge about the disease and effects of
complicated Diabetes Mellitus

To increase the communitys awareness as to the proper use of community resources to decrease the incidence of DM, treat and
manage its complications.
