Environment: - Purpose of environmental study is to mainly control the emission and pollution to
protect the environment.
Accidents: - Accident is an unexpected, unplanned & uncontrolled event. This can be result in injury to
person and damage to property.
Incident: - An event or chain of events, which has caused or could have caused personal injury,
damage to property or environment
(A dangerous occurrence includes both accident and Near miss cases. In other words all accidents
and near miss cases are incidents.)
Duties of a safety officer:- Identify the hazards & take or recommend preventive measures.
Root cause for the Accidents:Substandard act

(Unsafe act)

Substandard condition

(Unsafe condition)

Unsafe Act: - A human error or an act that can lead to an accident. Tool miss use
Unsafe condition: - The condition which can lead to an accident. Excavation with shoring shielding
Hazard: - Something that can cause harm to people or property.
Purpose of safety: - The purpose of safety is to save the life and protect the property.
Risk: - Risk means something low or high potential of hazard can cause harm to people or property.
Near miss: - An incident happened which results no human injury &No property Damage.
Common work place Hazard.

How can we prevent/control Accidents?
There are three types of control.
1

1. Engineering Control,
. Design or Redesigning, Quality of materials
2. Administrative control,
.Planning, Control, Organization
3. Proper PPE (Personnel protective equipments)
. PPE is working as barrier between human & accident.
Fire:Fire is chemical reaction between fuel, oxygen & heat producing light, heat and smoke. The fire
Triangle identifies 3 elements of a fire.
Fuel

Oxygen

Heat

Classification of Fire.
Class APlastics

Flammable solids

-

Wood, Paper, Cloth , Rubber,

Class B-

Flammable Liquids

-

Petrol, Kerosene, Gases

Class C-

Electrical Fire

Class DMetals.

Metal Fire

-

Energized Electrical Equipments
-

Magnesium, Other Flammable

Prevention is based on eliminating or minimizing one of the components of the “Fire Triangle”
Fire Extinguisher:

Extinguishing agent eliminates one of the vital elements of the fire by
Removing heat by cooling (or)
Breaking the chain reaction by smothering

Types of Fire Extinguisher
1. PRESSURIZED WATER
-Red
. Class A – wood, paper, cloth, Rubber, plastics
. Extinguishes by cooling material below the ignition point
2. CARBON DIOXIDE (CO2)
-Black
. Class B or C- Petrol, Kerosene, Gases, And Energized Electrical Equipment’s
. Extinguishes by Smothering burning materials
. Effectiveness decreases as temperature of burning material increases.
3.

4.

COMBUSTIBLE METAL
. Class “A,” ”B,” or “c”. Fires.
. Extinguishes by smothering burning materials.

MULTIPURPOSE DRY CHEMICAL
-Blue
. Class “A”, “B”, or “c” Fires
. Extinguishes by smothering burning material
5. FOAM
. Class “A”,”B”, and “K” fire
. On Class “K” fires, don’t use until after fixed extinguishing system has activated.

H2S is measured by PPM It is hazards gas Nature of H2S is toxic and flammable It is soluble in water It is a byproduct of crude oil H2S is a giant gas If the PPM is 10 a person can work for 8 hrs (Threshold limit value) If the PPM is above 15 stop the work and evacuate The densely is more than air so it will accumulated at lower level The smell of H2S is like rotten egg and it is color less H2S LIMT 1. .

Attendant should have attendance sheet of entrances 8. Entrant should write the name on the attendance sheet before entry 6. Risk assessment. Entrant should watch each other 16. Without confined space attendants presence no entrant should stay inside any confined space. He should ensure the entrant reach safety in confined space 17. Attendant should monitor continuously to ensure that the entrant is safe inside and communicate with entrant 18. Should be proper PPE as required ( Gas mask . Inspect the access before going and while going 12. 5. He shold know the hazardous inside 11. Respirator like SCABA) 9. Entrant should aware of PULL CODE SYSTEM ( COMMUNICATION) 11. then the line will be partially isolated . Remove from the source (ELECTRICAL) Partial isolation Positive isolation Passing Blinding . For isolating we need to close the valves.ISOLATION – Separation.insert nitrogen into a line Inerting – purpose of inerting is to displace the oxygen from the line Oxygen level needs to be below – LEL LEVEL 5 % LEL LEVEL 14 % WILL STARTS FIRE LEL LEVEL 23% ENRICHMENT LEVEL TIE IN: . ISOLATE:We can do TIE in by isolating the pipe line. If it is large line we need to isolate the contents into a tank and make sure the line is empty FLUSHING & STEAMING:12 . We need to insert spade between flanges for blinding. The purpose of TIE IN to replace a damaged part or add a new connection on existing pipe line. Once the line is totally blinded it is positively isolated. DRAINING:We need to drain the contents in the pipe line through the drain valve. still there is chances for passing through the valves even it is closed .process of inserting in a line is called blinding Spade Flushed – Inert – Purge -.

grinding. HOT WORK:Before starting any hot work we need to check the gas level and make sure it is in the permissible range.Flush water through the pipe line and wash out the sledge deposit in the pipeline. INERTING NITROGEN GAS:The purpose of inerting nitrogen is to remove or displace oxygen from the container. Once the nitrogen purging into the pipe line. Use high pressure steam for clean out the pipe line. again we need to do the gas test. LEL should be 0 or bellow 1. We need to do the gas test again and once the level goes below 5 % we start cold cutting COLD CUTTING :We can do the cold cutting by pouring water continuously on the cutting point and we can remove the damaged part and get ready for hot work. Then we can do the hot work like beveling. it will reduce the oxygen % . We need to do purging. welding and complete the TIE job PIGGING :The purpose of pigging is to remove wastes and sledge from the pipe line in fact cleaning we can do the pigging by using the balloons by pneumatic pressure the balloon to other end while moving the balloon will swipe the sludge and wastes to the other side of pipe line which is open Education - Gaining New Knowledge 13 . And the length of pipe is long the gas level Will be vary in this condition we need to do use the PLUGGING PLUGGING :We can do the plugging method by using the fire retardant balloons and we can isolate the gas contents in th long pipe line while doing the hot work. GAS TEST:We can do the gas test through the vent pipe or there is no vent pipe near the tie in area we need to drill a small hole and insert the probe Multi Gas Detectors and check the LEL & Oxygen level inside the pipe line. Otherwise if there is still gas above the required level. If the Oxygen level below 5% and LEL level below 10% we can do the cold cutting.
