Additional Comments: Overall S.H. has good Cognitive skills, Physical skills (gross & fine
motor), Math skills, Social Emotional skills, and she has good Language skills. She is
emerging as a four year old should be at this age. I have no concerns about her
development at this time.

Childs Initials: S.H.


Teacher: Mrs. Weedman
Overall Progress: S.H. has made good progress since the beginning of
school until now. She was writing her name all in upper case letters
and she is now writing her name in lower case letters with the first
letter as an upper case. Her number counting has gotten higher
than the first of the school year. S.H. has never been in a preschool
setting so in the beginning she had a hard time with separation

when her mom would leave. Now S.H. has no problem and tells her
mom bye.

Intellectual: S.H. is able to state her full name. At the beginning of


the school year I asked her what her name was and she told me.
When S.H. met her peers she was able to tell them her name as well
as ask them what their name was. We had the Lions Club come to
our school and one of the volunteers ask S.H. what her name was
and she told the volunteer her full name. We had a hearing
screening that was done on all of the children and the school nurse
had asked S.H. what her name was and she told her, her full name.
S.H can identify upper and lower case letters. S.H. has shown this
when we do the ABCs. S.H. has pointed out letters to teachers that
are hanging in the room. S.H. has also demonstrated the knowledge
of all upper and lower case letters when she has been at the writing
desk and has used the ABC stencils. The teachers have sat with S.H.
to see if she knew all of the upper and lower case letters so this
information could be entered in GOLD for the tracking program that
is used on all preschool students. S.H. can write numbers. As a
small group this is something that we do every day. S.H. has
demonstrated this daily. S.H. likes to go to the writing desk and use
the stencils. These are new to the classroom and she has stated
that she likes them. She uses the number ones and traces them and
then she will write them on her own. S.H. is able to print her own
name. She has been observed doing this. She writes her name
sometimes a few times a day. This is something that was observed
once again for GOLD to keep track of her progress. Her printing has
improved in comparison to the first day of school until just recently.
S.H. is able to recognize her own name. She demonstrates this by
finding her name with her peers and checking herself in when she
comes to school and when she goes home each day. S.H. is able to
find her own name on her writing journal each day to write in it. At
the writing desk each child has a Ziploc bag with their name on it
that has lids with letters on them to put their name together and
S.H. is able to do this. In the classroom there is a helper chart with
jobs that each child has for the week and S.H. is able to recognize
her own name on it.

Social Emotional: S.H. is able to manage her feelings. She has


demonstrated this by seeking out a person or a special object like
her blanket or a stuffed animal. S.H. follows limits and expectations.
She accepts redirection from teachers. S.H manages classroom
rules, routines, and transitions with occasional reminders. S.H.
cleans up when it is time to do so.

Work Habits: S.H. sometimes will get distracted. She does this when
sitting at the table when we are writing names in the journal. S.H.
has a long name and she has said this is going to take forever
because my name is long. She shows frustration with the way she
holds the writing tool and the look on her face. S.H. shows that she
is persistent with the things that she has an interest in. She has
demonstrated this by revisiting the interest until she has figured it
out.

Language: S.H. is able to comprehend language. She responds


appropriately to specific vocabulary and simple statements,
questions, and stories. S.H. follows simple request like to throw her
trash in the garbage, and helping clean up. S.H. speaks clearly. She
is understood by most people when she is talking to them.

Gross Motor: S.H. has good gross motor skills. She has demonstrated
this while playing outside. S.H. moves purposefully from place to
place with control. S.H. can sustain her balance while walking on a
balance beam. S.H. can ride the bike and scooter and climb up and
down on the play structure and use the steps as well.

Fine Motor: S.H. has good fine motor skills. S.H. uses her fingers with
a three point grip when using writing tools. She also uses refined
and finger movements when playing with playdoh, stringing beads,
and cutting with scissors. S.H. has also demonstrated this skill in
the science center with using tweezers to pick up the plastic bugs.
S.H. has been observed playing in the kitchen using a bowl to
pretend that she is mixing something up to cook.
