SCHOOL TEACHER
1. Every teacher will be responsible for School Health
Program with respect to his / her class
2. One of the teachers will be declared as focal person for
this program by the head teacher and will be
responsible to maintain record of School Health
Program in coordination with SH & NS.
3. The teachers will get practical training at the time of
screening of students by SH & NS / Medical
Officers.
4. Trained teachers will perform random screening of the
students at their own and will share the record with
SH & NS at the time of visit.
5. In female schools, students will be examined in
presence of a lady teacher and preferably LHV /
female paramedics will accompany male SH & NS /
Medical Officers, at the time of half yearly
screening.
6. Complete screening of all students of a school by
teachers will be held at least twice a year with
approximately six months gap. The teacher can,
however refer any student to the BHU/RHC
throughout the year.
7. SH & NS will be present in the school at the time of half
yearly screening of the students.

MEDICAL SUPERINTENDENTS OF
TERTIARY CARE, DHQ & THQ HOSPITALS
1. Referred cases of students under School Health
Program will be examined and treated on priority.
2. Consultations, investigations and treatment advice of
referred cases will be done expeditiously,
completed preferably on the same day.
3. A record register of School Health Program will be
maintained in the hospitals for record keeping and
follow-up.
4. Monthly Reports of cases examined / treated will be
submitted to Executive District Officer (Health). The
report will consist of total number of students
examined along with name, residential address and
school name with diagnosis and advice.

REFORMS PROGRAM
1. All the reports from Program Director, DHDC will be
collected, compiled and analysed by PMU.
2. Feed back will be provided to EDOs (H), EDO (E),
District Governments and higher authorities.
3. Close laison will be maintained with Education
(Schools) Department, PRSP, NCHD, UNICEF and
other stakeholders.
4. Monitoring evaluation and supervision will be ensured
in collaboration with office of Director General
Health Services.

OPERATIONAL ACTIVITIES OF
SCHOOL HEALTH PROGRAM
1. Screening of Primary and Middle School students of
catchments area by SH & NS at least once in every
six month.
2. School Health & Nutrition Supervisor (SH & NS) will
prepare two types of reports on prescribed format.
