(Approached pt. in the dayroom where he was sitting alone at a table reading the newspaper.) Hello Josh. How are you doing today? (I leaned against a chair slightly to try to hide my nervousness).

1G (Looks up and studies my face.) I’m alright I guess. You look familiar- do I know you? (Looks at my nametag.)

My name is Kathy and I’m a student nurse from Southwestern. We met last week when you were admitted to the unit – I helped you with your meal tray when –

Yes it does. Most people call me Kathy. You look like you’re doing much better this week. I was hoping we could talk for a little while, 10-15 minutes(Interrupts) About what? Well, since you’re doing so much better, I thought we could talk about the progress you’ve made over the past week. Laughs & folds the newspaper) So, you can stand up and present me as a case study to your classmates like they do in medical school? Actually, that would be fun. I certainly am an

Well , I’m not presenting a case study. (said nervously) what we talk about is confidential. If you happen to say something that I think the staff should know in order to help you, then I will share that information. (Smiling) yeah, well the staffs not too sharp around here. I think they have a combined IQ of 80. My IQ is 150, what’s yours? It has got to be higher than that nurse over there (points to the nursing station.) Actually, that nurse and the rest of the staff are quite competent. I’ve been very impressed with them. 6A

Yeah, well it doesn’t take much to impress a student nurse- I’d guess your IQ is about 110, 120, -am I close? Oh oh,(shaking his finger at me) from the look on your face I may have guessed too high.- It’s OK. – I’ll help you out with your psychology course.

I’m more interested in hearing about how you are doing. Can I sit down? Sure. – Go ahead – Shoot. Ask me anything you want. Last week, when you came in, I noticed you were pretty upset about someone being after you. (Laughs and leans a little closer to me) Didn’t you read my chart? Honey, someone was after me – Probably still is – Getting locked up probably

You think someone is still after you? For a student nurse, you’re not listening very well – (looks around the room surprisingly) I don’t “think” (makes exaggerated quotations marks in the air) I KNOW someone; more that one, is on my trail. Who is looking for you? People in Washington are looking for me (said in a halfwhisper, speech is becoming more pressured) I’d appreciate it if you would keep your voice down – I don’t want you to blow my cover. – I’m not worried about the lunatics in here – nobody would believe them anyways – But, if they interrogate you, I could be

in a lot of danger and so could you. They won’t stop looking until they find me. I –

Who are they? (Shifts chair a little closer to student nurse, and puts his arm on the back of the students chair) They work underground for the President. – It’s really the President who needs me. What does he want? (Scoots chair over even closer to the student) I have to go before nine judges on the Supreme Court. (Said nervously) You know what? I felt more comfortable talking to you when your chair was back farther. Would you move it back a little please?

We were going to talk about the progress you’ve made. Okay, Okay. – I’m not in restraints – I’m not getting shop up every two hours anymore. I take my medicine like a good little patient. (Pats self on the hand) Tell me about your medicine. What brand do you take? I take lithium carbonate 600 mg twice a day – whether I need it or not. What do you mean?

The corporation/termination game. – I take the medicine, behave like a good little boy, go before the Judge and look like the sanest person on earth, the judge scolds the psychiatrist for locking up such a goal-oriented upstanding citizen and he terminated our Dr. / patient relationship. There you have it same old same old. Get it! Yes, I understand what you are saying. (pause 5 sec) It must be hard on you to be in and out of the hospital so many times. Most times it’s unnecessary and a waste of time, but I’m on my feet and back to work within a couple weeks. Then I quit taking that useless medicine and 16A

I think you need to keep taking the medicine after you leave here, so you don’t have to keep going back to the hospital. The medicine has nothing to do with it! If people weren’t so jealous of me, they would leave me alone instead of trying to destroy me. (Agitated mildly hostile tone of voice) (This nurse interrupts @ this point saying the Dr. wants to see the patient in his office). Maybe we can continue our talk about your progress after you’re though with the Dr. He’ll probably discharge me after lunch. I’ll say goodbye before I leave. (Gets up and
