META PARTICIPATION AGREEMENTDisneys The Little Mermaid

THE UNDERSIGNED PERSON, being the parent and/or legal guardian of
___________________________________ (hereinafter Minor or
Participant) now hereby enrolls said minor in the META play production
of Disneys The Little Mermaid and states and agrees as follows:
1.
Tuition of $____________ ($200 if blank) is received and
acknowledged by META in exchange for the right to participate in all
activities and instruction in Disneys The Little Mermaid at the Lincoln
Theater in Mount Vernon WA.
2.
Disneys The Little Mermaid begins on Wednesday, January 4,
2016 and concludes on Sunday, March 26, 2016. Monday/Wednesday
sessions will begin at 6 pm. and conclude at 8 p.m. Saturday rehearsals will
begin at 11am and conclude at 1pm. Parent/Guardian is responsible for
transportation to/from rehearsals at St. Pauls Episcopal Church, Lincoln
Elementary School and the Lincoln Theater.
3.
All activities shall be conducted at the St. Pauls Episcopal Church,
Lincoln Elementary School or the Lincoln Theater unless otherwise
designated. Parent/Guardian consents to Minors participation in any and all
such activities.
4.
All activities contemplated herein shall be conducted under META
Supervision. Parent/Guardian states and acknowledges that some of these
activities, such as dancing, carry the risk of injury and releases holds
harmless META, St. Pauls Episcopal Church, Lincoln Elementary School
and The Lincoln Theater from liability for said risk. Parent/Guardian also
acknowledges that neither META, nor St. Pauls Episcopal Church, the

Lincoln Elementary School or The Lincoln Theater insures Participant for

the cost of medical care for such injuries.
5.
Participant shall report to designated META personnel at the time set
forth above remain under META supervision until Parent/Guardian appears
to retrieve him/her at the end of daily activities. META will employ a Sign
In/Out sheet to keep record of Participants whereabouts. Participant may
be excused from META, without refund, for cause as absenting
himself/herself from META activities or supervision. Participant may be
excused from META activities by accumulating excessive tardy days or
absences. Excessive is to be determined by the META leadership team
depending on age of child, circumstances, role in show and other variables.
6.
Parent/Guardian states and represents that Participant is fit and able
for the activities contemplated herein and will advise META in writing of
any health restrictions or certain needs of Participant, including, but not
limited to, allergies and diet restrictions.
7.
Parent/Guardian further agrees that META may photograph and/or
videotape training activities and performances by Participant and gives
his/her consent to META, and the Lincoln Theater to use such images and
videography without further consideration for purposes including, but not
limited to, training, promotion, and advertising.
