My daughter is 4 Mo. Her birth weight was 2.6

Hello doctor, I am a mother of 2 months old


baby boy. Since the time of birth my baby is on

my 4 month old baby is very irritated while

Hi mam.
I delivered a baby girl on 23rd march 2016. she

Try to feed on demand instead of feeding as per timings. Research the


various signs of hunger online and keep an eye out for those before you
start feeding your baby. Sometimes mothers tend to start feeding
babies much before they are actually hungry, thus leading to the baby
getting distracted during the feeds and not having a full and proper
feeding session. The best indication that your baby is getting enough
milk is that your baby should be putting on the required weight as per
the paediatricians instructions and growth chart.

As per the World Health Organization (WHO), infants should be


exclusively breastfed for the first six months of life in order to achieve
optimal development and health. Post 6 months of age, infants can
start receiving complementary foods in addition to breast milk.
After one year breastfeeding can be continued for another year if
mutually desired by the mother and her infant. One of my clients has
fed her baby for 2.5 years too. All the best : )

One of the main rules of breastfeeding is to feed from BOTH breasts at


every single feeding session. Your baby may have found a preference to
your left breast but you have to make sure to feed from both breasts at
each feed so that your baby gets a proper intake of fore and hind milk
and your breasts remain the same size. For now what you can do is try
to start your feed on the right breast, so your child tends to feed
properly especially since he will be hungry during the start of the feed,
and then move to the left breast. Dont worry about breast cancer, it
has nothing to do with this.
Always remember Demand = Supply. So the more you feed, the more
milk is produced by your body. Stimulation of the nipples and the
breasts is the key.
If you still feel that your baby is not getting sufficient milk, try to use a
breast-pump in between feeds for added stimulation.

Since your milk supply is adequate at night, try to pump out the extra
milk at night itself and refrigerate until morning. As I always say
Demand = Supply. So the more your girls stimulate your breasts the
more milk will be produced by your body. Try to feed them from both
breasts at each feeding and continue expressing in between feeds too.
If you still feel its not enough I would suggest a personal consultation to
look into the matter in depth. All the best : )

As per medical science, the best way to increase milk production is by


feeding exclusively or by stimulation of the nipples / breast.
My advice would be to reduce the intake of formula for your baby and
increase the number of breastfeeding sessions so that your supply will
gradually increase.
Along with this there are also certain traditional foods used by our
ancestors to increase the milk production through the generations.
Few of them are Fenugreek Seeds (Methi), Fennel Seeds (Saunf), Garlic,
Green Leafy Vegetables like Spinach and Sarson Ka Saag, Bottle Gourd
(Lauki), Dals & Pulses and Oats (Daliya).
Eat Healthy and Eat Wise : )

Breastfeeding the baby in a lying down position always has its own set
of risks, which is why you have to be extremely careful during the feed.
Remember to make sure the baby is turned to the side and is not on
his/her back. Also make sure the baby burps well in between as well as
after the feed. And most important Make sure you dont fall asleep
during the session.

Just as we remain healthy with the food we eat, its the same with your
nursing baby. The flavor of the breast-milk tends to constantly change,
depending on what your intake is. So in case your baby is sensitive to
anything that you are eating, it will show. Keep an eye on her reactions.
Try to restrict eating items with ajinomoto, types of fish with a high level
of mercury, and of course keep a tab on your caffeine and alcohol.
For the itching around your nipples, you need to consult your general
practitioner / paediatrician.
Enjoy your trip : )

Try to increase your babys feeding time slightly, and continue feeding
from both breasts at each feed. As long as baby isnt cranky means he
is getting enough milk.
As for your party, please do try to avoid alcohol or limit it as much as
you can. Alcohol tends to pass freely into the mothers milk and
normally peaks about 60 minutes after consumption. Therefore I would
advice you to limit your consumption to 1-2 glasses and carefully time
your next feeding thus minimizing your babys exposure to the alcohol
consumed. Avoid feeding for 2-3 hours after 1 drink and 4 6 hours
post your 2nd drink.

As long as your paediatrician is happy with your babys weight gain,


you should not worry so much. There is absolutely no need to start any
supplements without their permission.
Also, please avoid the bottle. Your baby isnt happy with it either. And it
isnt going to help reduce the speed of the let down reflex in any ways.
Instead try to empty the breast a little by massaging it just before the
feed so that the reflex is controlled and the baby feeds well.
And please dont worry so much. Enjoy your breastfeeding time with
your little one.

I am extremely sorry for your earlier miscarriage. And congratulations


on your current pregnancy.
Do remember always that your body recognizes abnormal
chromosomes in a fetus and naturally does not try to continue the
pregnancy, as the fetus will not develop into a healthy baby.
The more stress you put onto yourself, the more you put yourself at risk
for problems. God has been kind and you have gotten pregnant again
so soon, so have faith that all will be well.
If you are still worried, do take a second opinion from another
gynaecologist. Since your 8 weeks, you should be able to hear the
babys heart rate soon enough.

For the swelling in your feet, you could reduce your salt intake and keep
your feet elevated during the day and while lying down.
With regards to your babys less movements, do a fetal kick count (Eat
something, lie down and count the number of movements for an hour.
) and kindly get in touch with your gynaecologist at the earliest with the
results.

Honey & water is not advisable. I would suggest you exclusively


breastfeed without any supplementation until 6 months at least.
You need to recognize the hunger signs in your baby and feed on
demand instead of as per timings. Sometimes when a mother puts the
baby on the breast too early, the baby isnt hungry enough to have a
proper feed.
Feed on one breast for 10-15 minutes, burp her and then feed her for
however long she wants on the second breast. Incase she does fall
asleep, try to stimulate her a little bit, but if shes in deep sleep, then
await the next feed. For your next feed start with the breast you left off
with in the prior feed.
Also please coordinate with your paediatrician to know if your babys
growth is adequate to her age.

Extremely sorry for your last miscarriage and Congratulations on your


current pregnancy.
Unfortunately cervical incompetence is something which you and I have
no control on whatsoever. It could happen to anyone. Might I suggest
you discuss the options of complete bed rest and a cervical cerclage
with your gynaecologist. I too am currently pregnant and suffered from
cervical incompetence in my 4th month. With the cerclage, and Gods
Grace, I am close to my 7th month with my cervix tightly closed.

It is a very good thing that you are still breastfeeding your baby until 7
months. Yes, you will be tired as your workload and responsibilities
have increased, but unfortunately I will not be able to give you advice
on iron and calcium tablets. For this you will need to consult your
general practitioner.
Your paediatrician shall be able to give you a growth chart for your
babys age and weight.

It is said that exclusively breasfeeding is ideal until 6 months post which


you can start solids. If your baby is still enjoying his breast-time, I would
advice you to continue feeding and not replace his feeds with solids. A
6 hour stretch of sleep during the night is good.
The benefits of breastfeeding are always higher than solids / outside
foods. So try to continue as much as you can.

Congratulations on your pregnancy. Of course you can handle it. Keep


your mind strong and your body will automatically become stronger.
If your baby girl is having just one feed in the night, I dont think that
will affect your current pregnancy in any way. But I do suggest that you
take a second opinion from another gynaecologist so that your doubts
are cleared.

I will need to understand your situation in depth to know why your baby
was bottle fed from birth as normally that is not required. Currently the
main priority is to reduce the bottle off completely and bring the baby
back to the breast. For this I will need to see you personally. The more
you feed with the bottle the lesser your milk supply will become. Please
do not delay anymore as your baby has already been away from the
breast for the last 10 days.

The very fact that your baby is irritable while feeding means that you
could be having a wrong latch or holding her wrong. Please research the
various holding positions like Cradle, Cross Cradle etc and then use the
one that the baby and you are both are the most comfortable with.
Walking while feeding is going to tire you out unneccessarily.
With regards to her poor weight gain, sweating and the shivering of the
hands, you will need to consult your paediatrician.

Unfortunately I cannot help you with advice on this as it is not a part of


my practice. You would need to consult your gynaecologist.
As your tummy increases in size, the pressure on the back increases
thus resulting in discomfort. Try using a wedge pillow that gives good
support to your back, and also ask your husband to pamper you with
some massages. Sometimes a hot water bag also works. Just be careful
of the temperature.

As you have rejoined work and are relying only on expressed milk,
please do some research on storing milk for longer periods. You can also
consider purchasing an electric breastpump, so it reduces the amount
of effort put into expressing milk every single time and increases the
amount of milk being expressed. Do not worry, with dedication and
persistence, you will be able to feed exclusively for 6 months.

You could consider consulting your paediatrician in order to gradually

nerve which could have gotten pressed. You will need to consult your
doctor on the same and take adequate tests to find out what exactly is
the problem in detail. This level of pain close to the hips in the 4th
month of pregnancy is rare

In case you feel your baby is continuously crying at one particular time
every day, please do check for colic, as when colic does strike it strikes
at approximately the same time every day. Once you rule that out, you
can consider expressing milk in the afternoons so that in case you feel
your production is less in the evenings, you could supplement the feed
with the milk you expressed in the afternoon. Demand = Supply when it
comes to breastfeeding. The more you feed the more milk gets
produced.

All babies are different and take their own sweet time to learn how to
start sitting / walking etc. Thus you should not compare your baby to
other babies of his age group. He will definitely learn to sit up on his
own soon enough. But if in case this is really stressing you out, then do

What you intake during pregnancy has absolutely nothing to do with


your milk production post delivery. If you are not liking milk, please
dont force yourself to have it. Since you have twins you will have to
really put in a large effort while breastfeeding as it is going to take you

your sonography report will mention the approximate gestational age of
your pregnancy so you could continue counting from there too. But the
best and most convenient method is to download an app onto your
phone which helps you with all details. Some examples are What to
Expect, Baby Center, Baby Berry etc

You need to make sure your baby is hungry before you start feeding
him. Research on hunger signs to double check the hunger patterns.
Always feed on demand and never as per the clock. If your baby is
hungry he should feed for more than 5-10 mins for sure.
Also please make sure that you are feeding him from both breasts so
that he gets a mix of fore milk and hind milk at all feeds. If he still does
fall asleep while feeding try to stimulate him a little by putting him
down, tickling his ears etc so that he gets at least 10-15 mins on one
breast and another few minutes on the other breast. If your supply was
less he would not sleep. Instead he would be cranky for milk so dont
worry about your supply.

Firstly what are the reasons that you feel that your baby is not getting
sufficient milk? If he is cranky, not gaining weight or your paediatrician
is worried, then yes, you need alternatives. But if he is gaining weight
as per his growth chart and is a generally happy baby, then you need
not worry. Yet if you do need to increase your supply then try to feed
more often. The more the stimulation to the nipples the more the milk
production increases.
If healthy foods and ayurvedic tablets are not helping, then you may
consult your paediatrician or general practitioner for proper lactation
inducing medications. Alternatively if you want to try increasing the
production naturally, keep expressing milk and massaging your breasts
at regular intervals. This kind of stimulation does help as well.

For your weight gain, you will need to consult your gynaecologist as he
will be well versed with your pregnancy history. As long as the baby is
growing as per target you need not worry. I myself am 24 weeks
pregnant and have lost 14.5 kgs since conception and not gained
anything. All depends on your eating habits. If your baby is getting the
required nutrition and your baby is growing well, then please dont
allow this stress to bother you. For your gastric problems and acidity,
eat small regular and healthy meals every 2 hours, increase your intake
of liquids and be active with your walks.

Please do not worry. It is not necessary for colostrum to start leaking


during pregnancy. The whole process of labor induces your breasts to
start working double time, so you will definitely have enough colostrum
for your baby once he or she comes.
For now enjoy the last few weeks of your pregnancy. And remember to
feed your baby within the 1st one hour of their life. WITHOUT FAIL.
Also in the first 24 hours feed your baby every 1.5 -2 hours to stimulate
your breasts to increase its milk production. Post 24 hours, feed on
babies demand but make sure you feed from both breasts at each
feeding.

Refluxes are very normal for babies. But if he is vommitting a lot then I
suggest you consult his paediatrician for medication if required.

From what you are saying, I doubt you have less milk supply as your
babys weight gain is quite good too. Drinking for 5 mins every 2-3
hours is a concern, so try to massage your breasts before the feed so
that they are stimulated enough and it increases the let down reflex.
Expressed milk can be kept in room temperature for 1-2 hours, but with
this humid climate Id rather advise you to use your fridge, store the
milk there for a few hours and then reheat it in a bottle warmer. There is
no harm with pumping. In fact pumped and feeding expressed
breastmilk is much better than starting on formula.
With regards to your engorgements, dont worry if your breasts are not
engorged. It doesnt mean there is less milk. Just keep feeding
regularly, expressing and massaging your breasts. With this stimulation
the baby should get enough supply.

I will need to understand your breastfeeding pattern in indepth detail in


order to help you at this stage. Unfortunately since 3 months your milk
supply must have been reducing considerably taking into account the
fact that the baby was being supplemented with formula. It is definitely
possible to slowly and gradually bring the baby back to breastfeeds
only and eliminate the formula but I will need to meet you personally
for that.

My suggestion would be to continue breastfeeding exclusively for


another one month at least instead of starting cerelac so early. But if
that is not possible then try to maintain the stimulation of the breasts
by expressing milk during the day and by breastfeeding for longer
durations. In terms of intake you could consider natural methods like
healthy foods, green leafy vegetables, dals & pulses, fenugreek seeds
and even ayurvedic lactation inducing tablets which your general
practitioner can prescribe.

with an IUD post your C-Section as it is not a part of my practice. You
would need to consult your gynaecologist.
What I can tell you is that IUDs do have a considerable success ratio if
done properly. But always do remember that eventually it is an external
object being put into your body so there are certain risks and side
effects that come along with it. Please do have a detailed discussion
with your gynac and then make a decision. You could also consider
slightly more natural methods like ovulation dates, protection etc in
order to avoid a pregnancy until you are ready.

problem, as long as you have them in limits and dont end up falling
sick.
