PAYMENT INSTRUCTIONS

1. Present this statement when paying at any BDO Unibank, Inc. branch counters. The machine validation will serve as your proof of payment.
2. Please pay the Amount Due/Minimum Amount Due as indicated in this statement on or before your Payment Due Date to avoid late payment
charges.
3. For check payments*, please make checks payable to “BDO Unibank, Inc.”. Indicate the following at the back of the check: a) Account Name,
b) Account Number / Card Number and c) Contact Number.

Foreign Exchange based on the prevailing foreign exchange rate of MasterCard and BDO
Conversion Rate respectively at the time of posting. The service fee may be imposed at
the sole discretion of BDO and may be subject to change.
Installment
Acceleration Fee 5% of the total remaining balance or P300, whichever is higher
the next cycle date.
NOTE: Interest rates are determined by BDO and are subject to change in accordance with prevailing market rates. In case of conflict between the provisions of these reminders and the Terms and
Conditions for the Issuance and Use of BDO Simple Installment Credit Cards, the latter shall prevail.

IMPORTANT REMINDERS: 1. Applicable MEIR will be imposed on the account on the next cut
off date if: a) No payment was made on the Card on Payment Due Date, or b) Payment made
was less than the Total Outstanding Balance stated in the Monthly Billing Statement (MBS).
2. Cardholders are encouraged to call BDO Customer Contact Center from time to time to
check his/her available Credit Limit to avoid declined transactions or other inconveniences.
You may now avail of the Cash Advance facility! Avail up to 30% of your assigned credit limit
at any ATM bearing the MasterCard logo. Call the BDO Customer Contact Center to request
for your Cash Advance Personal Identification Number (PIN). For the complete Terms and
