Academic Improvement Action Plan- SLC’s 2017

Directions: In a separate document, complete one of these sheets for each class
in which you are receiving a C- or lower, including LINK if you do not have a
mentor aligned. Email your completed action plan(s) to your advisor, your
parent/guardian(s) and the teacher of the class.

A. Reflect on why you are underperforming in this class.

B. Write a specific and measureable goal related to this class.
Ex. Finish Chemistry with a B- or higher.

C. Brainstorm steps and potential obstacles in achieving your goal.

Ex. Steps:
a. Attend Steve’s office hours every week for at least 30 minutes,
regardless of what is going on at that moment in Chemistry.
b. Switch into a Help SIG
c. Work on chemistry each evening from 4-4:30pm regardless of what is
going on at that moment in Chemistry.

Ex. Obstacles:
a. Too much work in my other classes.
b. Not being able to adapt to a new schedule.
c. Other appointments that interfere with office hours/set work time.

D. Show your plan to your parent/guardian and solicit input from him/her/them
for ways to enhance your Academic Improvement Action Plan. Record his/her/their
ideas.

E. Email your final action plan(s) to your advisor, your parent/guardian(s) and
the teacher of this class.
