AFFIDAVIT OF UNDERTAKING

I, DARWIN VALDEZ OMBRETE, of legal age, married, Filipino, and a resident of

Sta. Cruz Village, Carmen, Cagayan De Oro City Philippines do hereby declare and
undertake the following:

1. That I am the same DARWIN VALDEZ OMBRETE who cause the execution of this
affidavit;

2. That I am the driver of a Mitsubishi Strada (Pick-up) more particularly described as

follows;

3. That I was recently at fault in driving one of the vehicles in an accident. The two
vehicles were on the process of traversing intersection, with (v-1) traversing along
Sales Street coming from (Lm-1) heading to Lm-2, while (v-2) traversing along
Monteverde Street coming from Suazo Street heading towards Gempesaw Street
and upon reaching aforesaid intersection, the front right side portion of (v-1),
bumped by the front side portion of v-2, as a result both vehicles incurred
damages.

4. That I undertake that I will claim my license and pay if there is necessary
administrative fine. Also, that I am willing to face any criminal or civil case that
will be filed against me;;

5. I am executing this affidavit to attest to the truth of the foregoing facts for
whatever legal purpose it may serve.
