TECHNICAL odontographs PURPOSE: To improve the management of tooth prin ts in order to provide standardized information, protect the legal interests of users and the responsibilities of health professionals and the dental service es tablishments. II. OBJECTIVES: 1. Standardize the chart and the basic nomenclatur e for recording of findings in the odontogram, which allow dentists to handle th e same information. 2. Establish criteria for recording data on the anomalies an d pathologies of the teeth and the use and management of tooth prints, in variou s production units of dental services, can be used in clinical, legal, forensic, statistical, research or teaching. SCOPE: This technical standard is mandatory for dentists in public and private institutions and the private practice through out the country. • • • • • • • • • • V. LEGAL BASIS: 1993 Constitution of Peru. Law No. 26 842, the General Health Law. Law No. 27 604, which amends the General Health Law in Articles 3 and 39. Supreme Decree N º 014-2002-SA, which approved the Regulations of the General Health Law. Law No. 27 604, Law of the Ministry of Health. Supreme Decree 023-2005-SA approving the Regulations of organization and functions of the Ministry of Health. Ministerial Resolution No. 776-2004/MIN SA Technical Standards adopted by the clinical history of Health Facilities in t he Public and Private. Law No. 27 878, Labour Law Dentist. Supreme Decree 016-20 05-SA which approved the Regulations of the Dentist Act. Code of Ethics and Prof essional Conduct of the clinics in Peru. III. IV. GENERAL PROVISIONS: 1. The odontogram should be part of the Summary of Dental an d Medical History. 2. The numbering system for odontogram should be two, or bina ry digit system proposed by the International Dental Federation and accepted by the World Health Organization. 3. The odontogram be developed individually for e ach patient during the first dental appointment and will be unchanged. 4. In par allel, should develop a odontogram to register the evolution of dental treatment . 5. In the initial odontogram must register only observed in the time of examin ation and should not sign the treatment plan. 6. Each record is made in the odon togram must respect proportionately the size, location and method of the finding s. 7. For the recording of finds in the odontogram only used the colors red and blue. 8. In the boxes corresponding to the teeth where you specify the type of treatme nt is recorded in blue stands when treatment is in good condition and in red whe n you are in poor condition. Any processing time will be recorded in red. 9. In the area of specifications should explain, identify, clarify with the findings t hat individuality can not be recorded graphically. 10. In the event that a tooth this more of an anomaly, these should be recorded in the item specifications. 1 1. The radiographic findings should be entered in the odontogram. 12. The graph in odontogram provisions of this rule is unique, and must be printed in black. T he crown must be at least an inch square and the root will be proportional to th is. (See Annex II). 13. The odontogram must be completed without emendations or deletions. For any change to occur by treating the professional responsible must record and sign the modification made to the item specifications. 14. Dental sp ecialties may add other classifications related to his field, but not modify or contradict well established by this rule. 15. The odontogram should be developed in a maximum time of 10 minutes. VI. SPECIFIC PROVISIONS: 1. The record of the findings will be made through the following notation: 1.1. Fixed orthodontic app liances square draw a cross inside, at the apices of the teeth which correspond to the ends of the orthodontic appliance, combining both squares with a straight line. The drawing will be blue when the unit is in good condition and in red wh en you are in poor condition. Specifications will be detailed in the type of app liances found. REMOVABLE orthodontic appliances will draw a line in zig-zag blue to match the tips of the maxillary teeth in treatment and this should be red wh en the unit is in poor condition.€Specifications will be detailed in the type of

appliances found. CARIES should draw the following carious lesion shape in the tooth surfaces will be fully committed and painted red. FINAL CROWN will draw a blue circle, which encloses the crown of the tooth presented by this treatment. In the corresponding box enter the symbol of the type of crown in capital letter s and blue. The central level through the Directorate General of Health of the People, will ensure the dissemination and monitoring of compliance with the above technical standards. 2. The disas and DIRESA as regional health author ities, will be responsible to implement, operationalize, monitor and supervise t he implementation of the Technical Standard odontographs. 3. The dentists of the various production units of dental services will be responsible for recording a nd proper use of tooth prints. FINAL PROVISIONS: 1. Institutions should dissemin ate this rule through their representatives, who participated in the development of tooth prints. 2. The implementation of this standard must be accompanied by training and information to staff of the dental service establishments. 3. Failu re to comply with the provisions of this rule will be punished according to exis ting administrative arrangements, the General Health Law and the Code of Ethics and Dental College, subject to any civil or criminal incurred. 4. The process of implementation of this standard will be progressively until six months after th e approval. VIII. The graph odontographs be as follows: IX.
