Brief Job Description of Trainee:

The Program expects each Trainee to demonstrate at all times desirable qualities
and traits as indicated in the performance Evaluation instrument. Please rate the
Trainee according to the scale indicated below. A total score of 75 or better is
considered passing and below 75% is failing.

96-100 Trainee has demonstrated at all times

87-95 Trainee has demonstrated most of the time
78-86 Trainee has demonstrated sometimes
75-77 Trainee has demonstrated rarely
Below 75 Trainee has not demonstrated at all


Completes all his/her tasks on time. He/she able to do more than one assignment in
his level at any one time. He/she demonstrates resourcefulness, versatility and
flexibility. Performs tasks with complete accuracy, thoroughness and neatness. The
quality output exceeds expectation. The output is impressive and within the
prescribed standards of the establishment or department.

Communication skills rating:

Possesses comprehensive knowledge of almost all phase of his/her work. Needs no
further instruction. Can work independently and with minimal supervision. Very
organized in his/her work. Plans and executes tasks in a systematic manner.
