Action Nursing Consideration _Assess patient for allergy to sulfonamides _Assess fo infection (Vital signs, apperance of wound, sputum, urine and stool; WBC) at beginning of and during theraphy. _Monitor intake and output ratios. Fluid intake should be sufficient to maintain a urine output of atleast 1200-1500 ml daily to prevent crystalluria and stone formation. _Monitor CBC and urinalllysis periodically during theraphy. May produce increased serum bilirubin, creatinine, and alkaline phosphatase _Inspect IV site frequently. Phlebitis is common. Patient Teachings _Instruct patient to take medication around the clock and to finish drug completely as directed. _Caution patient to use sunscreen and protective clothing to prevent photosensitivityreactions. _Advise patient to notifyhealth care professional if symptoms do not improve within a few days. _Advise patient to notify health care professional if skin rash, sore throat, fever, mouth sores, or unusual bleeding or bruising occurs. _Instruct patient to notify health care professional if symptoms do not improve within a few days.
