1. General Purpose:

required to administer business operation of the cooperative?
The cooperative will hire 1 full time manager to oversee the operations of
the cooperative and 2 clerks.

proposed cooperative will succeed in its operations.
a. Where and how will the cooperative obtain its operating capital? State
whether contributions/loans/donations/grants.
b. How much may the cooperative earn during the first year of business
operation?

f. How much will the Cooperative require for its operating expenses in the
First Year?
g. Does the Cooperative intend to have capital investment? If so, state what
capital investments and when such will be realized. No.
V. MANAGEMENT STRUCTURE:
d. What will be the role of the Board of Directors? Policy making body and

Note:
Remember that this is only a guideline; you may add, revise or omit items that are not
applicable to your proposed cooperative. The CDA reserve the right to conduct an investigation
into the proposed operations of the Cooperative. There is no need to submit project Financial
Statements and Cash flow for three years.
