Instructions to the Rater: Below you will find several factors important to the success of
the employee’s job. Use the following scale to rate the employees for each factor:

1 - Unsatisfactory: Employee does not meet basic job requirements and
immediate improvement is needed for employment to continue.

2 - Needs Improvement: Employee struggles to meet and does not always meet
basic job requirements. Improvement is needed to maintain current position.

3 - Adequate: Employee always meets expectations and basic job requirements.

4 - Exceeds Expectations: Employee always meets job requirements and often
exceeds them and/or completes them with superior quality.

5 - Outstanding: Employee always exceeds expectations, consistently
completing more objectives than assigned with superior quality, at the best
performance level possible.
