JOINT AFFIDAVIT OF
TWO DISINTERESTED PERSONS

We, ALFREDO C. EUGENIO and JERICO S. AGUSTIN, of

legal ages, Filipino citizens and residents of Barangay Sta. Maria,
Camiling, Tarlac, after having been duly sworn to in accordance with
law, do hereby depose and state THAT:

1. We both personally know PEDRO SIMON ALEGADO(Pedro,

for brevity),whowasborn on May 24, 1963 at Sta. Maria,
Camiling, Tarlac to Spouses SofronioAlegado and Alejandra
Simon;

2. Pedro is a member of the Social Security System (SSS) until

his death on August 3, 2015. Pedro died single and without a
child of his own;

3. Lately, when Dolores AlegadoRonquillo, the sister of Pedro,

went to the SSS Camiling Branch filed for the Death Claim
Benefit of her late brother, she was required to submit copies
of Pedros Birth record, CRS Form No. 4 (CENOMAR) and
Certificate of Death;

4. Upon submission of the required documents and upon

processing, it was discovered in the certificate of live birth of
Pedro that that the recorded Name of his mother appearing
therein is LEONARDA SIMON, instead of ALEJANDRA
SIMON;

5. Meanwhile, in the CRS Form No. 4 (CENOMAR) of Pedro, the

recorded name of his father appears as SOFROMO ALEGADO,
instead of SOFRONIO ALEGADO. Hence, the discrepancies;

6. We are not related to Pedro by affinity or consanguinity except

that of being his barangay mates;

7. We are executing this affidavit to attest to the fact that the

names SOFRONIO ALEGADO and LEONARDA SIMON, as
the parents of Pedro S. Alegado appearing in the above-stated
Certificate of Live Birth, SOFROMO ALEGADO and
ALEJANDRA SIMON, as the parents of Pedro Simon
 Alegadoas recorded in the above-saidCRS Form No. 4
(CENOMAR), and SOFRONIO ALEGADO and ALEJANDRA
SIMON, as the parents of Pedro Simon Alegadoappearing in
the above-stated Certificate of Death including in his SSS
record, refer to one and the same person, being the biological
father and mother, respectively, of our barangay mate Pedro;

8. Finally, we are executing this affidavit to attest to the truth of

the foregoing statements and for all legal intents and purposes
it may serve.

IN WITNESS WHEREOF, we have hereunto affixed our

signatures this 12th day of August, 2016 at Camiling, Tarlac,
Philippines.
