Nursing Intervention - asses for causative factors Rationale - to help determine possibilities of underlying conditions Evaluation After 3 hours of nursing intervention the client will be able to identify interventions to reduce risk of infection.


- Record and observe vital signs

– For baseline data

- Recommend routine or preoperative body care

- to reduce bacterial colonization

- Observe for localized signs of infection at insertion sites of invasive lines, sutures, surgical incisions/wounds -Educate client in infection control procedures

-To determine or lessen infection

-Reduce risk of crosscontamination
